As we continue to grow, we are looking for Licensed Practical Nurses (LPNs) to join our close-knit, dedicated team. Through our Pinnacle Plus program, we deliver personalized, pharmacy-led care directly into our clients’ homes—supporting medication management, monitoring wellness, and helping individuals maintain independence and dignity.

Your Role
As part of the Pinnacle team, you’ll provide in-home nursing care while working closely with a highly collaborative pharmacy and healthcare team.
Key Responsibilities:
- Deliver hands-on nursing care in clients’ homes, including medication administration (injections, oral medications, patches, inhalers, creams, eye drops)
- Monitor and assess patient health (blood glucose, blood pressure, pain assessments) and contribute to care planning
- Collaborate with pharmacists, physicians, nurse practitioners, and other healthcare providers to optimize patient outcomes
- Apply clinical judgment within LPN scope in alignment with policies and legislation
- Identify and document changes in health status, ensuring timely communication with the team
- Provide consistent updates and maintain strong communication with both the care team and clients’ families
What We’re Looking For
- Licensed Practical Nurse in good standing with BCCNM
- Strong communication skills and a collaborative mindset
- A compassionate, relationship-focused approach to patient care
- Ability to work independently while feeling connected to a team
- Valid driver’s license (company vehicle provided)
- Current CPR-C and First Aid certification
- Criminal Record Check (upon offer)
Schedule
- 8-hour daytime shifts (Monday through Sunday)
- Fair rotation of weekend shifts
- Full-time and part-time positions available
Experience
- New graduates and experienced nurses are both encouraged to apply
- Experience with diabetes management is an asset
