About The Role:
Moira Place Long Term Care Home is seeking a Nurse Practitioner (NP) to join our dedicated care team. In this role, you will work both independently and collaboratively to enhance resident outcomes, quality of life, and overall care.
As a clinical leader, you will support timely, resident-centred decision-making, contribute to best practice initiatives, and help reduce avoidable hospital transfers—ensuring care aligns with each resident’s goals and wishes. The NP will support the Medical Director and attending physicians with the on-call rotation. This is a meaningful opportunity to make a direct impact in a supportive, team-focused environment.
Key Responsibilities
- Provide comprehensive, resident-centred assessments, diagnosis, and treatment of acute and chronic conditions within NP scope of practice.
- Prescribe medications, order diagnostics, and monitor treatment outcomes, adjusting care plans as needed.
- Lead proactive, preventative care with a focus on reducing avoidable hospital transfers and supporting a palliative approach when appropriate.
- Manage complex geriatric needs including responsive behaviours, pain, infections, and chronic disease management.
- Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.
- Participate in admissions, ongoing clinical monitoring, and regular resident rounds to support continuity of care.
- Collaborate closely with physicians, nurses, and allied health professionals through care conferences and quality rounds.
- Provide clinical leadership, mentorship, and support to nursing staff, promoting best practices in long-term care.
- Collaborate with the AON Inc leadership team to assist in policy and procedure development
- Contribute to quality improvement initiatives, infection prevention programs, and compliance with Ministry and regulatory standards.
- Ensure accurate, timely documentation and participation in audits, inspections, and accreditation processes.
Qualifications
- Master’s degree in nursing (Nurse Practitioner Program) or BScN with NP certification.
- Current registration as a Nurse Practitioner (Extended Class) with the College of Nurses of Ontario.
- Eligibility to prescribe medications and order diagnostics in Ontario.
- Minimum 5 years RN experience and 2 years NP experience in long-term care, geriatrics, primary care, or community settings.
- Strong clinical judgment, leadership, and ability to work autonomously.
- Strong knowledge of geriatric syndromes, dementia care, and chronic disease management.
- Proficiency with EMR systems and Microsoft Office.
