Nurse Practitioner
Location: Community-Based, OntarioEmployment Type: Full-Time (37.5 hours/week, Monday to Friday)
About the Role
Our client is seeking a compassionate and experienced Nurse Practitioner to provide comprehensive primary healthcare services within a community health setting. This role focuses on delivering high-quality, patient-centered care to individuals experiencing acute and chronic illnesses, mental health challenges, and substance use disorders.
Working within an interdisciplinary team, you will assess, diagnose, treat, and support patients while promoting health, preventing illness, and improving overall well-being. Key Responsibilities
- Assess, diagnose, treat, monitor, and provide follow-up care for patients with acute and chronic health conditions.
- Conduct comprehensive client assessments and develop individualized, evidence-informed care plans.
- Promote health education, disease prevention, and chronic disease management.
- Collaborate within an inter-professional healthcare team to coordinate comprehensive patient care.
- Provide education and support to patients and families regarding treatment plans, medications, and self-management strategies.
- Offer telephone consultations and follow-up support as appropriate.
- Participate in peer reviews, chart audits, and quality improvement initiatives.
- Contribute to community education and outreach activities as opportunities arise.
- Support and mentor Nurse Practitioner and medical students through preceptorship opportunities.
Qualifications
- Master's Degree in Nursing with a Primary Health Care Nurse Practitioner Certificate.
- Current registration in good standing with the College of Nurses of Ontario (Extended Class).
- Membership with the Nurse Practitioners' Association of Ontario (NPAO) with professional liability insurance.
- Minimum of 3 years' experience practicing as a Nurse Practitioner.
- Demonstrated experience utilizing advanced nursing assessment and clinical decision-making skills.
- Experience working within an inter-professional healthcare team.
- Community health experience is strongly preferred.
- Pediatric experience is considered an asset.
- Excellent communication, interpersonal, organizational, and conflict resolution skills.
- Ability to work independently while managing a diverse patient caseload.
- Valid Ontario driver's licence and access to a reliable vehicle.
Working Conditions
- Full-time, Monday to Friday (37.5 hours per week).
- Occasional evening or weekend work may be required.
- Travel between community-based locations is required.
- Ability to work effectively with marginalized populations experiencing complex health, mental health, and addiction-related needs.
- Maintain accurate and timely clinical documentation.
Pay: $48.35-$68.48 per hour
