Exciting Opportunity as JFSC is expanding and looking for a Social Worker with Mental Health and Addictions experience.
Primary Duties and Responsibilities:
· Complete an initial assessment
· Develop a service plan and goals in partnership with the client
· Participate in interdisciplinary service planning and conferences by providing the team with current information and observations
· Measure clients' progress regularly and adjust services accordingly
· Assist with program development and implementation aligning with best practices
· Carry out case management functions including convening case conferences, liaising with community services and other professionals (especially education, social services, justice and health)
· Ensure the maintenance of accurate records in line with practice policy
Qualifications:
- A post-secondary diploma or degree in Human/Social Services, Addictions, Counselling, Psychology, or a related field
- Minimum 3 years’ relevant practical experience working with Seniors with Mental Health and/or Addictions
- Experience in an intensive case management role serving a vulnerable population, including individuals with trauma
- Excellent communication, organization, time management, and coordination skills
- Must have a positive and collaborative attitude, demonstrate flexibility, reliability and show initiative
- Non-violent crisis intervention and First Aid/CPR training are assets
- Class 5 driver's license, personal vehicle, clean driver's abstract and $2 million insurance liability
- Clear Criminal Record Check with Vulnerable Sector dated within three months of hire

Pay: $58,000.00-$64,000.00 per year
