You will be responsible for coordinating and providing quality care to clients in the community and will provide direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment.

Required Qualifications:
Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
Minimum of two years' experience within the last five years of Home Care experience, although skills and experience relevant to nursing practice in community sector will be considered. An excellent attendance record, reliability and personal suitability must be demonstrated through your employment references. Physically able to perform all assigned tasks and may involve physical exertion not limited to climbing stairs, walking outside, lifting, pulling, pushing, stooping, and carrying which may involve visits to client homes that are not ideal. Extensive experience providing case management services, with strong knowledge of Connect Care and service authorization process. Thorough understanding of Continuing Care Health Service Standards, with proven ability to integrate these standards into daily nursing practice.
Preferred Qualifications:
Clinical expertise in Home Parenteral Therapy (HPT), wound care, lower limb assessments, skin and ostomy care, and palliative care support. Skilled in delivering client education, empowering patients and families to manage health conditions effectively. Demonstrated ability to prioritize care effectively and manage time efficiently in fast-paced environment while maintaining composure and professionalism, ensuring safe and high-quality patient care.
