Job Description: Community Outreach Support Worker
Location: Based out of Bowmanville includes frequent travel outside the GTA also required.

Hours: 35 hours per week
Wage: $50,960.00 annual salary
2 weeks of paid vacation
Reports to: Outreach Supervisor Position
Summary of role: The Community Outreach Support Worker is responsible for providing trauma-informed, client-centred support to participants in our Outreach program. This role ensures a safe, respectful, and empowering environment for participants as they work toward healing, independence, and self-sufficiency. The worker will support participants in developing life skills, implementing coping strategies learned in therapy, and achieving personal goals. Additionally, this position maintains thorough documentation and collaborates with internal and external supports to ensure continuity of care.
Key Responsibilities
- Client Support & Case Management
- Conduct monthly 1:1 case management meetings with assigned Outreach clients
- Develop and review individualized case plans and goals
- Conduct weekly wellness checks and provide referrals and support as needed
- Assist clients with appointments, errands, and community-based tasks
- Provide safety planning and lifestyle awareness support
- Offer in-person support for Follow Care clients as needed to ensure a supported transition to independence Program Development & Facilitation
- Contribute to the development of Outreach programming and curriculum
- Facilitate monthly Outreach workshops and activities (therapeutic and recreational)
- Support creation of program tools such as safety kits and best practice guidelines
- Coordinate recognition of client milestones (e.g., birthdays, sobriety anniversaries) through gifts and personal check-ins
- Collect and track data to evaluate program effectiveness and identify emerging needs Collaboration & Community Engagement
- Liaise with therapists, doctors, addiction counsellors, and community supports
- Facilitate referrals to healthcare, education, housing, and other services Provide court support to clients as needed
- Maintain professional relationships with community partners and agencies
- Assist in community outreach, including delivering basic needs and meeting with clients in the field Administrative & Documentation
- Maintain accurate and timely client records; input notes within 24 hours of each shift
- Uphold privacy, confidentiality, and ethical standards in all documentation and client interactions
- Support collection and distribution of donations and care packages Other Responsibilities
- Participate in team-based approaches to problem-solving and service planning
- Travel outside of the region to meet client and program needs
- Perform other duties as assigned by the Outreach Supervisor Qualifications
- Diploma or degree in Social Work, Community Services, or related field, or equivalent experience
- Experience supporting vulnerable populations, including those facing homelessness, addiction, trauma, and systemic barriers
- Knowledge of trauma-informed care, harm reduction, and safety planning practices
- Excellent interpersonal, organizational, and communication skills
- Ability to work independently and collaboratively - Must have a valid G-class driver’s license and access to a reliable vehicle
- Recommended certifications: CPR/First Aid, Non-Violent Crisis Intervention (NVCI), trauma training Working Conditions
- Based in the Bowmanville Outreach Office with frequent travel
- Occasional evening or weekend hours may be required
