Description: The Social Worker II (SW) provides services to clients and families including psychosocial assessment, crisis intervention, education, treatment, advocacy, resource finding, consultation, and discharge planning. The SW II is responsible for the provision of services in accordance with established standards, and the Mission, Vision, Values, policies and procedures of Covenant Health. This position is covered by the terms and conditions of the HSAA Paramedical Professional/Technical Collective Agreement.

Required Qualifications:
Required Qualifications: Bachelor Degree in Social Work required. Active registration with the Alberta College of Social Workers (ACSW) required.
Additional Required Qualifications:
Knowledge of community resources, social programs, financial benefits, personal directives, and relevant legislation. Experience in assessment, care planning, and case management. Highly organized, with excellent written and verbal communication skills. Strong interpersonal skills and the ability to work collaboratively within an interdisciplinary team. Proficiency with Microsoft Office.
Preferred Qualifications:
Masters preferred. Recent healthcare experience, preferably in continuing care. Knowledge of capacity, consent, and guardianship legislation, with experience supporting patient and family decision-making. Experience providing information and referrals for housing, advance care planning, and end-of-life planning. Ability to provide short-term and grief counselling. Experience supporting caregivers through education, resources, and group facilitation.
