BACKGROUND
The Vaughan Community Health Centre (VCHC) is a not-for-profit, community-governed organization providing primary health care, social services, and illness prevention programs primarily to the residents of the Western and Northern York Region. We serve people experiencing barriers to accessing health care services due to culture, language, age, chronic illness, or the like. We are committed to providing services with and for local Indigenous, Black, racialized and other disadvantaged communities.
POSITION SUMMARY
The Foot Care Nurse works within the scope of the College of Nurses of Ontario to provide assessment and treatment of foot and lower limb ailments They are responsible for providing quality foot care services to help clients eliminate foot pain, gain mobility and maintain healthy feet. The Foot Care Nurse acts as a resource to clients and colleagues, can work independently and within a team, and is compliant with legislation and standards of practice. The Foot Care Nurse is a key member of the Centre’s allied healthcare team that works closely with primary healthcare and chronic disease management professionals for service delivery.
KEY ACCOUNTABILITIES AND RESPONSIBILITIES
- Conduct foot assessments/re-assessments, develop treatment and management plans, and maintain complete and accurate client progress notes in the clinic's EMR;
- Maintain approved cleaning and sterilization techniques for all instrumentation and tools used;
- Ensure that up-to-date infection prevention and control (IPAC) practices are followed as set out by regulatory and advisory bodies including but not limited to the College of Nurses of Ontario and PIDAC-IPC;
- Taking into consideration cultural background and the social determinants of health, educate and advise clients about the prevention and care of morbid conditions relating to chronic diseases like diabetes;
- Ensure continuity of client care through exchange of information and communication with the client, family members, and other service providers who may be involved with the client;
- Work as part of an interdisciplinary team, making referrals and providing support to VCHC clients;
- Work with Primary Care Team, Allied Health Team, Diabetes Team and Senior Chiropodist to develop, implement, and evaluate chiropody services (i.e. provide services to clients experiencing barriers);
- Make interdisciplinary referrals within VCHC team as appropriate to provide quality care to clients;
- Foot care services are provided during regular scheduled hours and may include home visits as required to VCHC primary care clients;
- Implement workflow processes in alignment with VCHC goals and objectives (i.e., health outcome data, alignment of documentation practices, etc.);
- Identify areas where the development of protocols and procedures are needed to improve client services or to promote more effective clinic functioning;
- Participate in Centre’s quality improvement initiatives for example, quality improvement plan, peer chart reviews, and other quality improvement activities as identified based on best practice guidelines and Vaughan CHC priorities;
- Maintain and develop professional competence through appropriate continuing education (i.e., peer interaction, literature review, conferences, courses, etc.).
QUALIFICATION REQUIREMENTS
- Bachelor of Science in Nursing (BScN) from an accredited institution
- Active registration and good standing with the College of Nurses of Ontario (CNO); and proof of professional liability insurance.
- Certified Foot Care Nurse (CFCN) with successful completion of an Advanced Nursing Foot Care course that is in alignment with the Canadian Association of Foot Care Nurses National Competencies for Advanced Nursing Foot Care in Canada
- Knowledge of wound care, lower limb circulatory diseases, and diabetes management
- Minimum of 2 years’ clinical practice experience as a Foot Care Nurse, including providing foot care services to people living with diabetes
- Excellent interpersonal skills and a commitment to inter-professional care, demonstrated ability to work effectively in a multidisciplinary team environment
- Strong clinical and effective time management skills in a high-volume setting
- Excellent health assessment skills, clinical judgment, analytical and problem-solving skills
- Experience working in an electronic health record system (EMR).
- Commitment to ongoing professional development and staying current with best practices
- Valid driver’s license, regular access to a vehicle and appropriate insurance for personal automobile
- The ability to communicate in a second language relevant to clients living in Western and Northern York Region is an asset.
