At Vision, the well-being of our residents and the satisfaction of our team are our top priorities. We are currently seeking a dedicated and compassionate Registered Dietitian to join our team on a temporary contract basis.
Reporting to the Administrator, the Dietitian's duties and responsibilities include but are not limited to:
· Screen/identify residents at nutritional risk, conduct (quarterly, annual, significant changes) nutritional assessments, and develop/modify individualized nutrition care plans.
· Review and address clinical-focused referrals daily, completing supporting documentation in PointClickCare.
· Assist with food service-related referrals as needed.
· Input resident weight data monthly.
· Review and address any significant resident weight changes monthly.
· Collect and input resident height data annually.
· Participate in interdisciplinary care reviews with verbal or written input or by delegating attendance to the Food Service Manager/Food Service Assistant Manager.
· Collect and analyze relevant nutrition statistics monthly.
· Attend Professional Advisory Committee meetings quarterly.
· Member of nursing care program team as applicable.
· Auditing (dining room, snack cart, IPAC) per VNH policy.
· Provide nutrition/dietary inservice education for dietary, nursing, and PSW staff.
· Biannual menu review and approval.
· Develop and review annually policies and procedures governing resident nutritional care and food safety, per best practice guidelines.
· Provide Food Service Manager support with the dietary team and resident admissions on an as-needed basis.
