Adult Diabetes Services at Trillium Health Partners supports over 15,000 patients annually through the Diabetes Management Centre (DMC) and the Centre for Complex Diabetes Care (CCDC).
We are seeking an experienced and passionate Clinical Team Lead to provide clinical and operational leadership within our Diabetes Management Centre. Centre for Complex Diabetes Care (CCDC)
The CCDC provides specialized, collaborative care for individuals with complex diabetes needs. Through an interdisciplinary approach, the team develops personalized care plans to support optimal health outcomes.
Diabetes Management Centre (DMC)
The DMC delivers comprehensive diabetes education and management services for adults living with Type 1, Type 2, and gestational diabetes, as well as individuals requiring diabetes support during hospitalization. Our team empowers patients through education, resources, and individualized care.
Position Summary:
Reporting to the Manager, Adult Diabetes Services, the Clinical Team Lead is accountable for supporting excellence in patient care, team performance, and day-to-day operations within the Diabetes Management Centre. The successful candidate will work closely with the CCDC Clinical Team Lead to ensure consistent, high-quality service delivery across the program.
This leadership role combines clinical expertise, operational coordination, staff development, quality improvement, and interdisciplinary collaboration to support exceptional patient experiences and outcomes.
Key Responsibilities:
Foster a positive, collaborative, and inclusive practice environment through mentorship, coaching, and role modelling.
Coordinate daily operations, staffing, scheduling, and resource utilization in collaboration with the leadership team.
Address clinical and operational issues while promoting safe, effective, evidence-informed care in collaboration with the leadership team.
Support equitable and standardized patient access in alignment with Ontario Health Service Agreements
Participate in the development and implementation of clinical protocols, processes, and program improvements.
Identify service gaps and recommend innovative solutions to enhance patient care and operational effectiveness.
Facilitate difficult conversations and support effective conflict resolution when required.
Act as a liaison to promote integrated, patient-centred care across the continuum of services.
Qualifications:
- Bachelor’s degree in a health related discipline
- Member of a regulated health-care profession, in good standing with respective College
- Minimum 3 years experience in a formal or informal leadership position within a healthcare environment
- Experience in areas of chronic disease management, diabetes management, case management or similar related experience
Certified Diabetes Educator (CDE) designation is considered an asset
Excellent communication, interpersonal, organizational, and problem-solving skills.
Ability to effectively prioritize competing demands in a fast-paced environment.
Demonstrated change management and quality improvement capabilities
Proactively identifies opportunities for program and service improvement, providing innovative recommendations that support quality outcomes and organizational priorities
Commitment to ongoing learning and professional development
Proficiency with Meditech, EPIC, Microsoft Office, and other clinical applications.
