Job Summary
We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. The RPN will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. Responsibilities
- Deliver comprehensive patient care in diverse settings such as long-term care, acute care, hospice, or pediatric environments.
- Administer medications accurately following physician orders and established protocols.
- Monitor patient vital signs and assess health status regularly to identify changes requiring intervention.
- Assist with medical procedures including phlebotomy, wound care, pain management, and laboratory specimen collection.
- Maintain detailed and accurate documentation using electronic health records systems such as Epic or Cerner.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Educate patients and their families about health conditions, treatments, and post-discharge care instructions.
- Ensure compliance with healthcare regulations, safety standards, and infection control protocols.
- Support patients with dementia, senior care needs, pediatric cases, or hospice requirements as applicable.
- Participate in case management activities to coordinate ongoing patient services and resources.
Requirements
- Valid registration as a Registered Practical Nurse (RPN) with the appropriate licensing body.
- Proven experience in nursing home settings, hospitals, acute care, or home health environments.
- Knowledge of medical terminology, medication administration protocols, and clinical procedures such as phlebotomy.
- Experience working with diverse patient populations including seniors, children, or hospice patients is preferred.
- Familiarity with electronic health record systems like Epic or Cerner is highly desirable.
- Strong communication skills with the ability to collaborate effectively within multidisciplinary teams.
- Ability to handle multiple priorities while maintaining attention to detail in fast-paced settings.
