Community Program Coordinator - Job Description
ACCOUNTABILITY OBJECTIVE
Under the direction of the Community Program Lead, support the implementation of engaging programs and activities designed to enhance the physical, social, and emotional well-being of participants within an exciting new community initiative focused on connection, wellness, and aging well in the community.
BACKGROUND QUALIFICATIONS
A recognized diploma from a community college relating to Personal Support Worker. Must hold a valid C.P.R. and First Aid course. Must have access to an automobile with an appropriate level of liability insurance coverage and full G license. Demonstrated leadership ability with comprehensive knowledge of community services. Maturity and good physical health, empathy, patience and tact are important qualities to possess. Computer literacy is needed.
KEY RESPONSIBILITIES
· Research and support the development of new and engaging activities and programs in collaboration with the Community Program Lead.
· Assist with the planning, setup, implementation, and cleanup of programs and activities.
· Provide support with personal care and activities of daily living for participants, as required.
· Assist with kitchen and meal-related duties as needed.
· Attend community outings and external programs to support and assist participants.
· Must be comfortable providing safe and supportive transportation for participants to and from the home, as required, using the company vehicle.
· Promote a positive, collaborative, and respectful team environment within the department and throughout the home.
· Demonstrates flexibility and willingness to temporarily assist with the duties of other team members, as required.
· Actively participates in ongoing learning and professional development opportunities.
· Supports the orientation and mentorship of new PSW students and assists with supervision as needed.
· Interacts with participants, families, volunteers, and team members in a professional, courteous, and compassionate manner.
· Maintains and ensures the proper care and safe use of equipment utilized in program and service delivery.
· Act as a champion for the new Memory Clinic pilot initiative and participate in all required education and training related to the program.
WORKING CONDITIONS
Considerable standing, walking, bending, sitting and stooping. Exposure to client elements through regular contact. Stress resulting from dealing client needs and time pressures.
HEALTH AND SAFETY RESPONSIBILITIES
· Understands and follows all workplace health and safety policies and procedures.
· Performs all duties in a safe manner to help reduce the risk of injury to themselves, co-workers, volunteers, and clients.
· Remains alert to actual or potential hazards and promptly reports concerns to the Community Program Lead.
· Does not operate or use unsafe or faulty equipment and reports concerns immediately.
· Wears appropriate personal protective equipment (PPE) and clothing as required by the task, Safety Data Sheets (SDS), or organizational policies.
· Reports all workplace injuries or incidents to the Community Program Lead promptly and seeks first aid or medical attention as required.
· Participates in all required fire safety education, demonstrations, and fire drills.
