Job Summary
The Nurse Practitioner (NP) is an integral member of our interdisciplinary team and plays a vital role in supporting the health and well-being of the communities we serve by providing a range of primary care services to individuals of all ages. The NP will provide clinical support to community physicians, including solo physician practices in the Mississauga area, to improve patient access to care. This position requires regular travel between multiple clinical and community locations and involves a high degree of independent practice while fostering strong collaborative relationships with physicians, staff, community partners, and other stakeholders at the sites being supported. In addition, the NP will provide leadership in the development, implementation, and evaluation of SFHT programs and quality improvement initiatives to enhance patient care and service delivery.
Duties and Responsibilities
· Practices autonomously while working as part of an interdisciplinary team providing periodic health examinations, pharmacological and counseling interventions, health promotion, and chronic disease management.
· Builds and maintains strong collaborative relationships with physicians, both within Summerville and in the community, staff, community partners, and other stakeholders across supported practice sites to promote integrated, coordinated, and accessible patient care.
· Provides comprehensive and excellent health care to patients of Summerville, as well as patients of affiliated community physicians, in accordance with standards set by the College of Nurses of Ontario (CNO), SFHT guidelines, and in collaboration with other care providers in a patient-centred manner;
· Provides assessments, diagnoses and holistic care for patients of all ages;
· Demonstrates a leadership role within the SFHT by coaching, guiding and mentoring other team members AND/OR leading the development and/or delivery of new or improved programs;
· Participates in quality improvement initiatives including planning, implementation and reporting;
· Participates in program development, implementation, evaluation, and reporting as needed;
· Maintains accurate, concise and confidential EMR documentation pertaining to patient care and ensures ongoing communication with the care team;
· Provides clinical coverage and continuity of care across multiple practice locations in the Mississauga area, adapting to the unique needs of each site while ensuring consistent, high-quality, patient-centred care.
· Engages with community partners to plan and/or deliver services with the goal of improving the health of people in our community (e.g. initiatives led by the Mississauga Ontario Health Team).
Qualifications and Skills
· Master’s Degree in Nursing from a recognized Post-Secondary Institution;
· Minimum of 3 years working experience in a primary care setting;
· Current registration with the College of Nurses of Ontario as a Nurse Practitioner – Primary Health Care;
· Liability Protection through the Canadian Nurses Protective Society (CNPS);
· CPR Certification required;
· Intermediate knowledge of MS word, Excel and EMR (e.g. Accuro);
· Able to partner and work collaboratively as part of an interdisciplinary team;
· Good critical thinking and decision-making skills;
· Ability to use digital technologies for patient care and program delivery;
· Excellent written and verbal communication skills, including facilitation skills.

Pay: $60.60-$65.64 per hour
