Position SummaryAt Stem Health, we believe that exceptional care does not end at the appointment; it extends through every referral, every follow-up, and every moment a member is navigating a system that can feel complex and impersonal. Our Care Coordinator and Client Advocate is the person who makes that navigation feel effortless.
We are seeking a compassionate, clinically grounded, and highly organized individual to serve as the connective tissue between our members and the broader healthcare system. In this role, you will manage the full arc of care coordination from specialist referrals and diagnostic scheduling to care transitions and follow-through while building the kind of trusted, ongoing relationship with each member that is central to everything we do. This role is ideal for someone with a clinical or health navigation background who leads with advocacy, excels at follow-through, and believes that nothing should fall through the cracks.
Key ResponsibilitiesCare coordination and referral management-
Act as the primary liaison between members, specialists, hospitals, diagnostic labs, and community providers ensuring care transitions are smooth, timely, and well-documented.

Initiate, track, and follow up on specialist referrals, diagnostic orders, and clinical procedures from start to completion, ensuring no step is missed and every result is returned and reviewed.

Coordinate pre-procedure instructions, preparation requirements, and post-procedure follow-up for members undergoing investigations or interventions.

Proactively identify and resolve gaps or delays in care pathways, escalating to the clinical team when needed and keeping members informed throughout.
Client advocacy and relationship management-
Serve as each member's dedicated point of contact for all care-related questions, coordination needs, and ongoing health navigation building a trusted, consistent relationship over time.

Advocate on behalf of members within the healthcare system navigating wait times, provider availability, and administrative barriers with persistence, professionalism, and resourcefulness.

Conduct proactive follow-up calls and, where appropriate, external visits to ensure members are engaged, supported, and progressing along their care plans.

Listen actively and respond with empathy recognizing that care coordination is as much about emotional presence as logistical precision.
Care planning and interdisciplinary support-
Support the development and maintenance of individualized care plans in collaboration with the clinical team, ensuring plans reflect each member's current status, goals, and priorities.

Participate in interdisciplinary case reviews, contributing care coordination insights and flagging any member concerns to the appropriate provider.

Assist members in understanding their diagnoses, medications, care plans, and next steps translating clinical language into clear, actionable guidance.

Support members in setting health goals and staying on track with recommended care, acting as a consistent, encouraging presence throughout their journey.
Insurance, referral, and administrative navigation-
Support insurance-related processes including pre-authorizations, benefit verification, and coverage inquiries liaising with insurers on behalf of members where applicable.

Maintain a thorough understanding of Ontario's healthcare system, referral pathways, OHIP coverage, and available community and specialist resources.

Assist members in accessing publicly funded services, community programs, and external supports that complement their Stem Health care.
Documentation and clinical workflow-
Maintain accurate, timely, and complete records of all appointments, referrals, follow-ups, care plan updates, and member interactions within the clinic's EHR system.

Ensure care coordination documentation is always current and accessible to the clinical team, supporting continuity and safety across all providers.

Handle all member information with strict adherence to PHIPA privacy standards and clinic confidentiality policies.
Qualifications & Experience-
Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO) is strongly preferred; other relevant backgrounds include patient navigator, medical social worker, health coach, or private care concierge.

Bachelor of Science in Nursing (BSN) or equivalent post-secondary education in health navigation, social work, or a related health field.

Minimum 3 years of experience in care coordination, case management, community health, or outpatient clinical support.

Strong working knowledge of Ontario's healthcare system, including referral pathways, specialist networks, OHIP coverage, and community resources.

Exceptional communication and interpersonal skills warm, credible, and equally confident in clinical conversations and member-facing interactions.

Highly organized with outstanding follow-through nothing on your watch stays outstanding longer than it should.

Proficiency with EHR/EMR systems and digital clinical workflows. Use of Telus Health’s CHR is an asset.

Demonstrated commitment to member privacy, PHIPA compliance, and ethical health navigation.

Additional training or certification in chronic disease management, health coaching, or care navigation is a significant asset.

Bilingualism (French or other languages) is an asset in serving Toronto's diverse member population.
