An organization in partnership with Nova Scotia Health is seeking four (4) full-time Nurse Practitioners (NPs) to provide interim coverage from April through June 2026, supporting a Transitional Care Facility prior to permanent NP hires. This is a unique opportunity to practice in an innovative, restorative model of care, within a collaborative inpatient environment
About the Transitional Care Facility
- The facility supports adult patients (18+) in Nova Scotia’s Central Zone who: No longer require acute care (medically ALC with non-medical discharge barriers)
- Require additional time, services, and support before safely returning to the community
- Benefit from a restorative, activating environment focused on regaining and maintaining functional independence
Role Overview: Nurse Practitioner
- Provide full-scope post-acute inpatient care for 22–25 patients
- Be onsite Monday–Friday
- Establish Expected Date of Discharge (EDD) within 24–48 hours of admission
- Communicate discharge planning with families and the care team
- Diagnose and manage acute and chronic conditions
- Support smooth transitions to home/community through collaboration with the NSH Outreach Team
- Complete documentation in relevant EMR
- Collaborate with pharmacy partners on medication management
Ideal Candidate
- We are seeking NPs who:Are licensed and in good standing to practice in Nova Scotia
- Have experience in inpatient, post-acute, geriatric, or transitional care settings
- Thrive in collaborative, interdisciplinary team environments
- Are passionate about restorative care and supporting patients to return home safely
- Are available full-time for the full April–June contract period
