Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role offers an opportunity to work with diverse patient populations, including pediatrics, seniors, and individuals requiring hospice or dementia care. The LPN will play a vital role in supporting patient health and well-being through skilled nursing interventions and compassionate service.
Duties
- Administer medications and treatments accurately following physician orders and established protocols
- Monitor patient vital signs and assess patient conditions regularly
- Assist with patient admissions, discharges, and transfers
- Document patient care activities using electronic health records such as Epic or Cerner systems
- Perform phlebotomy procedures and laboratory tests as needed
- Provide wound care, pain management, and assist with mobility and personal hygiene
- Collaborate with healthcare teams to develop and implement individualized care plans
- Educate patients and families on health management, medication use, and post-discharge care instructions
- Support patients with dementia, senior care, hospice needs, or pediatric requirements as applicable
- Ensure compliance with safety standards and infection control protocols
Skills
- Proven experience in nursing home, hospital, or acute care settings is highly desirable
- Strong knowledge of medical terminology, medication administration, and case management principles
- Experience with electronic health record systems such as Epic or Cerner preferred
- Competence in specialized areas such as dementia care, pediatrics, hospice care, or pain management is a plus
- Excellent patient care skills with a compassionate approach to diverse populations including seniors and children
- Ability to work effectively within multidisciplinary teams in fast-paced environments
- Strong communication skills for patient education and team collaboration
- Ability to handle laboratory procedures like phlebotomy accurately and efficiently
- Knowledge of assisted living operations and medical office procedures is advantageous
