RN/RPN responsibilities include:
- Provide clients with primary health care services, including education, screening, patient assessment, healthy lifestyle support and chronic disease management
- Responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, and supporting factors that impact customer satisfaction.
- Administer medications and procedures per established policies and guidelines.
- Documenting observations, assessments and changes in patient’s condition; collaborating with health team members to facilitate positive patient care outcomes as well as providing a full range of practical nursing care to patients.
- Excellent critical thinking skills, with an ability to set priorities and work effectively in a fast-paced environment.
The ideal candidate will have:
- Registered Nurse or Registered Practical Nurse Certificate (CNO)
- Valid Vulnerable Sector Criminal Check
- Updated immunization record (TB test within 1 year)
- Proficient verbal and written English communication skills
- Flexible work schedule (Monday to Friday and Saturday to Sunday)
- Valid driver’s license and access to a motor vehicle required to work
