About the Role
The Social Worker – Life Links Program, provides client and family-centred services to support youth, (ages 9-19, or up to 21 if in school) in successfully navigating key transitions, including puberty, high school, and the move toward adulthood and community-based services. In addition, the Social Worker will also provide short-term service navigation for children between the ages of 7-8 and their families.
Working as part of an interdisciplinary team, this role focuses on empowering youth and families through a strengths-based approach grounded in the “F-Words” in child development: fun, fitness, function, friends, family, and future.
Key Responsibilities
- Support youth (ages 9–21) and families through ongoing Life Links services focused on transition planning, goal setting, self-advocacy, independence, and long-term coordination of care.
- Provide short-term Service Navigation support for youth (ages 7–8) by assessing needs, connecting families to appropriate community, mental health, school, and developmental services, and discharging once connections are established.
- Deliver supportive, goal-oriented counselling to youth related to emotional regulation, coping, family/school challenges, life skills, and transitions associated with a diagnosis or disability.
- Support and facilitate Life Links groups and programming at the centre and within the community as part of a multidisciplinary team.
- Collaborate with internal teams and community partners to ensure coordinated care, referrals, and access to appropriate resources and programs.
- Apply evidence-informed therapeutic approaches while maintaining accurate documentation and participating in intake, program delivery, case reviews, and evaluation.
What We’re Looking For
- Bachelor of Social Work (BSW) required.
- Registration (or eligibility) with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Minimum 3 years’ experience supporting children, youth, and families with complex or diverse needs.
- Strong knowledge of Chatham-Kent community services and supports.
- Experience with strength-based, family-centred approaches, and psychosocial supports
- Excellent communication, collaboration, and relationship-building skills
- Strong organizational, time management, and problem-solving abilities
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency with electronic documentation systems

Pay: $61,948.00-$77,079.00 per year
