Job Overview
The Odawa Native Friendship Centre (Odawa) is a non-profit organization serving the First Nations, Inuit and Métis community in the Ottawa-Carleton Region of Eastern Ontario and surrounding communities.
The Life Long Care (LLC) program services chronically ill, cognitively impaired, physically disabled, frail and elderly urban Indigenous people of all ages. The program provides transportation services; opportunities for socialization; service navigation and advocacy; and cultural, emotional and spiritual supports to promote independent living and good quality of life. The LLC Coordinator will organize and oversee the activities and transportation for the participants, as well as ensuring program criteria is upheld.
Responsibilities
- Provide case management, service navigation, advocacy, and referrals to support chronically ill, frail, elderly, cognitively impaired, and physically disabled Indigenous clients in maintaining independence and quality of life.
- Plan, develop, implement, and evaluate culturally appropriate programs, activities, and services that promote social inclusion, wellness, and community engagement.
- Conduct client assessments, maintain accurate records, collect program data, and prepare reports in accordance with organizational requirements and funding guidelines.
- Coordinate transportation services and assist clients in accessing medical appointments, community resources, social supports, and essential services.
- Facilitate educational workshops, support groups, and wellness activities that incorporate Indigenous cultural teachings, traditions, and healing practices.
- Collaborate with internal programs, community agencies, healthcare providers, and Indigenous service organizations to ensure comprehensive and coordinated client support.
- Provide emotional, cultural, spiritual, and practical support to clients while promoting dignity, self-determination, and successful aging within the community.
Requirements
- Minimum 2-year College Diploma in Health Studies and/or Social Service, or a combination of an equivalent and relevant education, skills and abilities as determined by the employer.
- 3-5 years of demonstrated and related work experience with the elderly, frail, physically disabled and/or chronically ill.
- Knowledge of community support services both Indigenous and mainstream.
- Strong knowledge and awareness of Indigenous culture, and both historical and contemporary Indigenous issues.
- Ability to develop, implement and deliver program and services.
- Knowledge and experience in supporting individuals so they main remain in their community and maintain a quality of life respective of their independence and dignity.
- Strong communication, listening and interview skills, as well as critical thinking and problem-solving capabilities.
- A strong foundation of Indigenous teachings and ceremonies to promote a foundation for growth, health and healing.
- Must have excellent written, verbal, time management and organizational skills.
- First Aid/CPR certification required or be willing to be certified.
- Posses a valid G driver’s license, satisfactory drivers abstract, access to a reliable vehicle and be willing to travel and transport clients/participants when required.
- Ability to obtain a satisfactory CPIC – Vulnerable Sector within 30 days of employment state date as a condition of the employment offer.
