Purpose of Position:
To provide support for individuals who receive services under the INS Outreach Program. These programs support individuals who meet the criteria for Developmental Disability, Acquired Brain Injury, or Personal Supports Initiative (PSI) These individuals have a high need for self-determination and will work with the requirements for accessing supports. Services will be delivered in a culturally sensitive manner in the community of Dawson Creek to enhance the social, emotional, psychological and physical quality of life for individuals served.
Scope of Position:
Reporting to Independence Networking Services, Inc. Risk Reduction Coordinator; this position will deliver support services for individuals/families that promotes independence, social participation, and well-being for individuals to live full and meaningful lives of their choosing. Program delivery involves services that maintain or improve the physical, social, and psychological health of individuals who, for a variety of reasons, may not be fully able to complete all aspects of daily living independently.
Responsibilities:
1. Home and Community - Deliver services at the community level
Activities
- Facilitates supported individual interaction to increase/enhance community access
- Maintains open communication with community resources, professionals and agencies (within the bounds of confidentiality)
- Interacts with others involved with referred individuals to support their personal relationships within the community
- Assist families/relatives in providing support or providing assistance for the individual to make appropriate living plans, resolve personal adjustment issues, or cope with other difficulties
- Participates as part of an interdisciplinary team to provide services
2. Implement Support Services
Activities
- Ensure that all activities are implemented in a culturally sensitive manner.
- Ensure that services are designed to enhance/maintain skill level and to encourage independence and safe choice making.
- Develop goals as established with supported individual’s team (PATH and Person Centered Goal Plan).
- Provide skills teaching and support for individual as outlined in their Person-Centered Goal Plan
- Involve individual in activities in the community identified to meet needs (including transportation) using strategies consistent with Circle of Courage
- Where applicable; assist individual in developing a schedule that includes participation in social skills, community access, and wellness activities
- Keep a record of all contacts with individual/family outlining progress and barriers, for each visit. Complete written progress reports as requested
- Follow through with strategies in place for the individual that will enhance skill level and increase quality of life. These strategies must be introduced with sensitivity and discretion.
- Ensure procedures are followed according to established standards and align with INS Mission Statements and policies and procedures (i.e., CARF, CLBC)
3. Participate as an INS Team Member
Activities
- Interacting and cooperating with other members of the care team
- Documenting and reporting any abuse of the supported individuals
- Carrying out all duties in a manner which provides motivation for supported individuals
- Documenting, reporting and planning of activities for supported individuals independence, social participation, and well being
- Documenting and file management - adhere to guidelines and procedures
- Act as a participating member of individual's team fostering a positive relationship with all members.
- Participate in annual performance reviews - clarifying career goals; participate in identification of personal training needs.
- Attending and participating in training workshops.
- Preparing personal work plans including goals and objectives.
- Attend all relevant meetings as requested by INS Inc.
Qualifications:
Education and Knowledge:
Grade 12 education
Training and Experience:
One-year recent related experience (Minimum 1 year experience supporting individuals with a developmental disability)
or
Equivalent combination of education, training, and experience
