Kincardine Family Health Team
The Kincardine Family Health Team (FHT) is a group of allied healthcare professionals with a mission to provide comprehensive patient care utilizing an interdisciplinary approach that is responsive to the needs of the community.
We are seeking an experienced, qualified individual to join our team in the new role of Director of Clinical Operations (Expansion Sites), reporting to the Executive Director. This role leads the development, implementation, and monitoring of operational policies, protocols, and procedures to support the efficient delivery of high-quality patient care. Key responsibilities include managing interdisciplinary team members and partnering with physicians, clinic managers, and internal and health system partners. Working collaboratively across sites, the Director of Clinical Operations establishes operational performance standards and mutually agreed-upon expectations that promote safe, effective care. The role also evaluates and reports on internal processes to inform decision-making and contributes to a culture of learning, quality, and sustainability. The Director of Clinical Operations works with team members and partners across the organization to ensure services align with Ontario Health objectives and FHT’s mission and strategic direction.
Roles and Responsibilities:
· Builds and maintains positive working relationships with physicians, clinic managers, and team members by establishing clear, reasonable, and mutually agreed-upon expectations that align with our vision and mission.
· Collaborates with team members to optimize patient care delivery processes, resolve operational problems, and develop policies and procedures as required.
· Provides leadership and management to eight FHT team members, and growing, working at expansion sites with Family Health Organizations (FHO), supporting the co-design of programs and services. This includes supervising, coaching, and managing the performance of interdisciplinary health professionals (IHPs).
· Fosters a work environment that exemplifies integrative thinking, change management, quality improvement, interdisciplinary and ethical practice, and professional autonomy in decision-making.
· Collaborates with all members of the FHO physician group and administration, and with FHT IHPs, to ensure accountability, operational efficiency, and effectiveness.
· Leads team-based meetings to foster communication and gather input in support of a collaborative approach.
· Keeps the Executive Director informed of day-to-day operations in a timely manner.
· Coordinates onboarding for new team members.
· Coordinates time away (vacation, personal time, sick time, etc.) to ensure clinical leadership coverage is maintained.
· Supports the achievement of FHT’s strategic and operational plans, driving improvements in organizational performance and system integration.
· Ensures organizational compliance with relevant professional practice standards (e.g., regulatory colleges), legislation (e.g., PHIPA, ESA, OHSA), and organizational policies and procedures.
· Participates in Ontario Health reporting activities.
· Manages human resource processes (e.g., hiring, performance management, vacation requests, and continuing education requests) in collaboration with the Human Resources Manager.
· Maintains fiscal accountability by collecting, monitoring, and analyzing fiscal and utilization data. Promotes effective use of resources by monitoring budgets, identifying variances, and implementing corrective actions as required to remain within approved expenditures.
· Provides consistent, timely responses and follow-up to employees and health system partners.
· Liaises with internal and health system partners to align and integrate services across and within the populations served.
· Ensures outcome measures are in place for services in collaboration with IHPs, Quality Improvement Decision Support Specialist, and the Data Management Coordinator.
· Works collaboratively with the team to prepare and implement a Quality Improvement Plan.
· Contributes to the development of the Annual Operational Plan.
· Maintains current knowledge of Primary Health Care priorities and programs in the Province of Ontario and ensures all programs and services align with evolving priorities.
· Provides clinical guidance and expertise to ensure services meet the needs of community residents and Ontario Health’s goal of increasing capacity for attachment of new patients.
· Collaborates with IHPs to identify professional development activities to support learning needs and enable team members to work to their full scope of practice.
· Participates on committees as required.
· Maintains equipment inventory and ensures a maintenance schedule is followed.
· Oversees registration and tracking of all clinic inventory in case of theft, damage, or upgrades.
The above responsibilities are not to be considered all-inclusive. Other related duties may be assigned to meet the operational needs of the FHT.

· Proficiency in a Microsoft 365 environment (Word, Excel, PowerPoint, and email) and experience with electronic medical records (EMRs).
· Strong verbal and written communication skills to effectively communicate with team members and health system partners.
Education/Experience:
· An undergraduate degree in a health profession, health sciences, or health administration or equivalent.
· Membership with a healthcare regulatory college.
· A minimum of 3 years in a progressive leadership role involving direct supervision of health care providers preferred, or demonstrated equivalent experience and education.
· A demonstrated commitment to ongoing learning.
· Experience with digital health technologies (e.g., Accuro EMR and Ocean) is considered an asset.
· Experience working in a primary care setting is considered an asset.
· Valid Ontario driver’s license and access to a reliable vehicle.
Work Locations:
· The Kincardine FHT is a multi-site organization, with team members working at the various locations based on organizational/coverage needs. This position requires capacity for on-site attendance at multiple clinic locations on a weekly basis.
