JOB PURPOSE/SUMMARY
Within the standards of nursing practice established by the College of Nurses, will provide medical support to all seniors in the Extended Care Unit and ensure their physical, mental, emotional and spiritual needs are being met.
QUALIFICATIONS
Education:
- College 1 to 3 years. Registered and in good standing with CNO and RPNAO.
Skills and Abilities:
- Must have First Aid/CPR.
- Training in Palliative Care, Dementia, Alzheimer's, Gerontology and Diabetes.
- Excellent communication and interpersonal skills.
- Sensitivity and understanding of Indigenous issues and provide a trauma informed approach to health care.
- Demonstrated leadership skills with ability to effectively act as lead on shift coordinating client care.
- Anishnaabemowin an asset.
Experience:
- 1 to 3 years in long term care/working with seniors.
- Experience working with/in First Nation communities an asset.
Other:
- A Vulnerable Sector Screening is required for this position.
KEY JOB FUNCTIONS/RESPONSIBILITIES
- Work shifts in ECU as needed.
- Coordinate and monitor care plans and ensure quality of care for ECU residents.
- Conduct, chart/document all assessments, observations, physician's orders, update care plan and care provided for all HCC clients.
- Administration and management of medications, nursing procedures & treatments, wound management, monitoring for therapeutic interventions etc.
- Contact physician and other health care providers as required.
- Monitor activities of clients to keep them mobile and as physically and mentally fit as possible.
- Coordinate work of PSWs on shift.
- Assist with departmental orientation of new employees and in-service education in the ECU.
- Promote a healthy environment with clients, staff and visitors at the Extended Care.
- Conduct daily check in and report to next shift.
- Maintain open lines of communication among staff, supervisor and families.
- Maintain a system of internal control and management reporting.
- Receive and replace all shifts for call-ins at the ECU.
- Must be able to work 12 hour shifts at the extended care unit (day & night shifts) when required.
