The Fort Frances Family Health Team is looking for a full-time Patient Navigation & Health Information Coordinator.
The PNHIC acts as a bridge between individuals and the healthcare system, guiding patients and their families through complex processes, coordinating services to address various needs, including physical, social, emotional, and practical ones. The Patient Navigation & Health Information Coordinator will be working as part of an inter-professional health team and will support patients by facilitating access to care and resources. The PNHIC will provide one on one direction, patient education, and will facilitate support, including completing the required paperwork.
Skills and Qualifications
- Registered Nurse preferred; consideration will be given to other regulated health professionals and/or relevant experience
- Must maintain registration with the applicable professional college, liability insurance, and current CPR/First Aid certification
- Experience in health navigation, primary care, chronic disease management, and prevention services
- Demonstrated advocacy, critical thinking, communication, organizational, and computer skills, with the ability to work both independently and collaboratively, manage competing priorities, and effectively use electronic medical record (EMR) systems
- Knowledge of regional healthcare systems and community-based resources
- Ability to build effective relationships with patients, families, healthcare providers, and community organizations
- Flexibility in hours of work to provide extended hours of service
- Ability to lift to 50lbs under reasonable circumstances
- Valid driver’s license and access to a vehicle
- Current Vulnerable Sector criminal records check
