The successful candidate will work closely with our physician and interdisciplinary team to deliver high-quality, evidence-based, patient-centred care to individuals and families across all stages of life.
Responsibilities
Nurse Practitioner (NP)
- Provide comprehensive primary care, including assessment, diagnosis, treatment, and management of acute, chronic, and complex health conditions.
- Order and interpret diagnostic tests and prescribe medications within the NP scope of practice.
- Develop and implement individualized care plans in collaboration with patients, families, the physician, and the interdisciplinary healthcare team.
- Provide preventive care and health promotion, including immunizations, well-baby, well-child, well-woman, and chronic disease management.
- Conduct medication reviews and monitor treatment plans.
- Coordinate care with specialists, hospitals, community agencies, and allied health professionals.
- Maintain accurate and timely documentation using the PS Suite electronic medical record (EMR).
- Participate in quality improvement initiatives and support evidence-based practice.
Registered Nurse (RN)
- Perform patient assessments, triage, and nursing interventions within the RN scope of practice.
- Assist the physician with examinations, procedures, and clinical care.
- Administer medications, injections, immunizations, and treatments as ordered.
- Provide patient education regarding medications, disease prevention, chronic disease management, and healthy lifestyle choices.
- Monitor patient progress and communicate changes to the healthcare team.
- Coordinate follow-up appointments, referrals, and patient recalls.
- Maintain accurate and timely documentation using the PS Suite electronic medical record (EMR).
- Ensure compliance with infection prevention and control practices, privacy legislation, and clinic policies.
Qualifications
Nurse Practitioner (NP)
- Current registration in good standing with the College of Nurses of Ontario as a Nurse Practitioner (Extended Class).
- Master's degree in Nursing or completion of an approved Nurse Practitioner education program.
- Current Basic Life Support (BLS/CPR) certification.
- Strong assessment, diagnostic, prescribing, and clinical decision-making skills.
- Experience in family medicine or primary care is preferred.
- Excellent communication, organizational, and interpersonal skills.
- Experience using the PS Suite electronic medical record (EMR) is preferred.
Registered Nurse (RN)
- Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse.
- Current Basic Life Support (BLS/CPR) certification.
- Previous experience in family medicine or primary care is considered an asset.
- Proficient in clinical assessment, patient education, medication administration, and immunizations.
- Strong organizational skills with the ability to manage multiple priorities in a busy clinical environment.
- Experience using the PS Suite electronic medical record (EMR) is preferred.
- Excellent communication and teamwork skills.
