Overview
We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role offers an opportunity to work with diverse patient populations, including seniors, children, and individuals requiring specialized care such as dementia, hospice, or acute medical needs. The RPN will collaborate with multidisciplinary teams to ensure comprehensive and personalized care plans are delivered efficiently and empathetically.
Responsibilities
- Administer medications and treatments accurately following physician orders and established protocols
- Monitor patient vital signs and assess health status regularly
- Assist with activities of daily living (ADLs) such as bathing, dressing, and mobility support
- Document patient information accurately using electronic health records systems like Point click care
- Provide wound care, including dressing changes and wound assessments
- Support patients with dementia, hospice, or chronic conditions by implementing specialized care plans
- Collect laboratory specimen collection when required (urine, swabs)
- Educate patients and their families about health management and treatment options
- Collaborate with physicians, therapists, and other healthcare professionals to develop and update care plans
- Ensure compliance with safety standards, infection control protocols, and privacy regulations
Skills
- Leadership skills
- Proven experience in nursing home, hospital, or acute care settings is highly desirable
- Strong knowledge of medical terminology, medication administration, and patient care procedures
- Experience with electronic health record systems such as Epic or Cerner is preferred
- Skilled in managing dementia care, senior care, hospice support, pediatrics, or home care environments
- Excellent communication skills for working effectively with patients of all ages, including children and elderly populations
- Compassionate approach with a focus on patient-centered care and pain management strategies
- Capable of case management duties when necessary
- Familiarity with assisted living facilities or medical office environments is a plus
