REST Centers is a B3 (Black-led, Black-focused, Black-serving) charity dedicated to
eradicating youth homelessness in the Peel Region. We are seeking a passionate and skilled
Case Manager to join our Bridge of Hope Program. This role is central to our mission of
providing transitional housing and holistic support services to youth (ages 16–24) experiencing or at risk of homelessness, particularly those transitioning from the foster care system.
Position Summary
The Intake & Youth Services Coordinator serves as a key point of entry for youth seeking
services through REST Centres. The Coordinator is responsible for receiving and reviewing
referrals, connecting with prospective clients, conducting intake and eligibility assessments, and determining the most appropriate REST program or service pathway based on each youth's needs and circumstances.
In addition to leading the intake process, the Coordinator supports the day-to-day operation of REST's Brampton Centre by creating a welcoming and youth-friendly environment, assisting youth and visitors accessing the Centre, coordinating youth communications and engagement activities, and providing administrative and resource support to program staff.
The successful candidate will be highly organized, compassionate, responsive, and able to balance youth-facing responsibilities with accurate documentation, assessment, communication, and administrative duties.
Key Responsibilities
1. Intake, Assessment & Referral Coordination – Approximately 50%
- Serve as a central point of contact for referrals and inquiries regarding REST programs and services.
- Review referrals received from youth, families, community organizations, child welfare, schools, government partners and other referral sources.
- Contact referred youth in a timely manner to complete initial screening and intake.
- Conduct comprehensive intake and eligibility assessments in accordance with REST's program criteria, policies and procedures.
- Identify presenting needs, immediate concerns, housing circumstances, risks, strengths and appropriate service pathways.
- Determine which REST program or service best aligns with the youth's needs and eligibility.
- Facilitate internal referrals and warm handoffs to appropriate REST staff and programs.
- Where REST is unable to provide the appropriate service, support connections and referrals to relevant community resources where appropriate.
- Maintain accurate and timely intake records, referral information, consent documentation and other required client information.
- Maintain and monitor referral and intake waitlists and conduct required follow-up.
- Track intake trends, referral volumes, eligibility outcomes and service needs to support program planning and reporting.
- Identify urgent or high-risk situations and escalate concerns to the appropriate program or management staff in accordance with REST protocols.
- Maintain confidentiality and ensure that all personal information is handled in accordance with organizational policies and applicable privacy requirements.
2. Youth Reception & Centre Support – Approximately 25%
- Provide a welcoming, respectful and youth-friendly first point of contact for individuals accessing the Brampton Centre.
- Greet youth, families, community partners and other visitors and direct them to the appropriate staff or service.
- Ensure youth and visitors follow established sign-in and site procedures.
- Respond to general inquiries regarding REST programs and services.
- Support youth with accessing approved practical resources, including the distribution and tracking of bus tickets and other designated supports.
- Assist youth who attend the Centre in connecting with the appropriate REST staff member or program.
- Support a safe, organized and welcoming environment within the Centre.
- Escalate safety, behavioural or urgent client concerns in accordance with organizational procedures.
3. Youth Engagement & Program Coordination – Approximately 15%
- Support communication and engagement with current and former REST youth.
- Coordinate and distribute e-blasts, program announcements, event invitations, reminders and other youth communications.
- Maintain up-to-date youth communication and distribution lists in accordance with consent and privacy requirements.
- Support program teams with outreach and registration for workshops, events and activities.
- Support the development and maintenance of REST's monthly program and engagement calendar.
- Coordinate with program staff to ensure upcoming activities, workshops, events and key dates are accurately reflected in the organizational calendar.
- Assist with basic event and program logistics as required.
4. Brampton Centre Administration & Resource Coordination – Approximately 10%
- Monitor office, program and youth resource supplies at the Brampton Centre.
- Maintain basic inventory records and identify when supplies and materials need to be replenished.
- Coordinate approved supply requests and communicate resource needs to the appropriate staff.
- Maintain organized reception and common areas.
- Provide reasonable administrative support related to the operation of the Centre.
- Assist with maintaining program materials, forms, resources and information available to youth and visitors.
Qualifications
- Diploma or degree in Social Work, Social Service Work, Child and Youth Care, Psychology, Sociology, Human Services, Community Services or a related field.
- Minimum 2–3 years of relevant experience working with youth, preferably youth experiencing or at risk of homelessness, housing instability or involvement with the child welfare system.
- Experience conducting intake, screening, needs assessments, service navigation or case coordination is strongly preferred.
- Working knowledge of data collection and client database systems, with experience using Salesforce (or a comparable CRM) considered an asset; ability to maintain accurate, timely, and confidential client records to support intake, case tracking, and funder reporting.
- Demonstrated ability to engage youth using a strengths-based, trauma-informed, and culturally responsive approach.
- Understanding of youth homelessness, housing instability and the social service system.
- Experience working with Black, racialized and other underserved youth and an understanding of the systemic barriers affecting their access to housing and services is an asset.
- Strong interpersonal and communication skills, including the ability to build rapport with youth experiencing complex circumstances.
- Strong organizational skills with demonstrated ability to manage multiple referrals, follow-ups and competing priorities.
- Excellent documentation and attention to detail.
- Comfortable using client databases, Microsoft Office/365, email communication systems and other digital platforms.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Ability to exercise sound judgment and recognize situations requiring escalation to clinical, program or management staff.
