Position: As an integral member of the primary care team, the LPN Panel Manager identifies and tracks panels for physicians, assists with the proactive care of patients and manages data and reports relevant to the practice population. The LPN Panel Manager assesses and communicates the need for patient health screening within the Electronic Medical Record (EMR) and plays a key role with regards to coordination of screening, referrals, and appointments for patients. This role is primarily technical but will have clinical nursing duties such as injections, patient medication reviews, and assistance with other clinical procedures in the primary care setting.
Education/Experience:
· Fully qualified/registered Licensed Practical Nurse (LPN)
· Fully licensed by CLPNA
· Experience in a healthcare setting
· Quality improvement experience
· Outstanding computer skills including data entry, standardization of protocols and running queries in databases. Strong computer skills in Microsoft Office (primarily Outlook, Word & Excel).
· EMR experience preferred.

Pay: $27.04-$36.31 per hour
