Leeds & Grenville Community Family Health Team - DIABETES EDUCATION CENTRE
Registered Dietitian (Permanent, Full-Time, 1.0 FTE)
Leeds & Grenville Community Family Health Team invites applications for a permanent full-time (1.0 FTE) Registered Dietitian. The successful candidate will work collaboratively with members of the interprofessional team to provide high-quality nutrition care to patients. Through assessment, counselling, monitoring, and follow-up, the Registered Dietitian (RD) will provide evidence-based nutrition therapy to support health promotion, disease prevention, and chronic disease management. The RD will establish patient-centred, goal-oriented treatment plans, evaluate outcomes, and adjust interventions as needed. The RD will also contribute to program delivery by identifying, coordinating, and managing resources and strategies that support optimal patient care.
Qualifications:
· Bachelor’s degree in Food and Nutrition or an equivalent degree, along with an accredited dietetic internship or equivalent supervised practical training. Post-graduate education is an asset. Current registration and membership in good standing with the College of Dietitians of Ontario.
- Certified Diabetes Educator (CDE) designation, or willingness to obtain.
- Demonstrated ability to assess nutritional status, identify nutrition-related concerns, develop nutrition care plans, and evaluate the effectiveness of interventions.
- Experience providing both individual and group nutrition counselling.
- Experience supporting patients with chronic disease management through nutrition interventions. Experience in diabetes education is considered an asset.
- Proficiency with electronic medical records and strong computer skills.
- Demonstrated critical thinking, problem-solving, and sound clinical judgment.
- Excellent organizational, interpersonal, and communication skills.
- Ability to work effectively both independently and as part of an interdisciplinary team.
- Valid Driver’s license and access to a vehicle
The successful candidate will work 37.5 hrs/week. The position requires that the successful applicant work at the Brockville and Gananoque sites as required. The position requires that the successful applicant work some evenings and weekends as required. Salary to commensurate with experience ($35.75 - $41.63/hr). This is a unionized position with SEIU Local 1 – Canada
