Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN-Staff will provide high-quality patient care across various settings, including nursing homes, assisted living facilities, hospitals, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively within a multidisciplinary team. The ideal candidate will have experience in acute care, dementia care, pediatrics, hospice, or senior care, and be committed to improving patient outcomes through attentive and personalized care.
Responsibilities
- Deliver direct patient care by monitoring vital signs, administering medications, and assisting with daily activities.
- Collaborate with registered nurses, physicians, and other healthcare professionals to develop and implement individualized care plans.
- Document patient information accurately using electronic health records systems such as Epic or Cerner.
- Assist with specialized procedures including phlebotomy, pain management, and laboratory specimen collection.
- Provide compassionate care for patients with diverse needs such as dementia, hospice, pediatrics, or senior populations.
- Educate patients and their families about health conditions and post-discharge care instructions.
- Ensure compliance with all safety protocols and infection control standards.
- Support case management activities by coordinating services and advocating for patient needs.
Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of practice.
- Prior experience in nursing home settings, hospitals, acute care, or home health is highly preferred.
- Knowledge of medical terminology and medication administration protocols.
- Experience with electronic health records systems such as Epic or Cerner is desirable.
- Strong patient care skills with a focus on dementia care, pediatrics, hospice, or senior care is a plus.
- Ability to work effectively in fast-paced environments while maintaining attention to detail.
- Excellent communication skills and a compassionate approach to patient interactions.

Pay: From $40.00 per hour
