We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our Assisted Living team as a Team Leader. In this role, you will provide high-quality resident-centered care while guiding and supporting care staff to ensure exceptional service delivery.
As a Team Leader, you will play a key role in enhancing health, wellness, dignity, and quality of life to our residents. You will collaborate with residents, families, healthcare professionals, and Fraser Health representatives to coordinate care that respects individual choices and promotes independence.
Key Responsibilities
- Provide Assisted Living services in accordance with individualized wellness plans and Fraser Health Funded Support Plans
- Administer and assist with medications, Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IDALs) and health assessments
- Complete initial and ongoing assessments of residents' health, wellness, and leisure status
- Develop, implement, evaluate, and revise individualized wellness and leisure care plans based on residents' needs and outcomes
- Provide wound care, catheterization, ostomy care, and other interventions using appropriate infection control and aseptic techniques
- Assist with residency planning by liaising with residents, families, Fraser Health, and community resources
- Maintain accurate records of nursing observations, interventions, teaching, counseling and resident care evaluations
- Report and document unusual occurrences following established procedures
- Participate in quality improvement activities, research initiatives and evaluation of resident care programs, policies and protocols
- Participate on committees related to resident care and workplace improvement
- Assign tasks in accordance with organizational Assignment and Delegation of Task policies and assess Team Personal Assistance (TPA) competencies
- Provide guidance, education, and work direction to Team Personal Assistants
- Maintain a safe environment for residents, families and staff by following established safety practices and emergency procedures
- Promote resident wellness, leisure involvement, dignity, respect, and individual choice through daily participation in scheduled and unscheduled programs
- Follow established protocols to problem-solve resident-specific issues and seek guidance from the Assisted Living Manager, Care Coordinator, or Senior Team Leader as required
Additional duties may be assigned as needed.
Requirements
- Current Licensed Practical Nurse (LPN) License with practicing membership in British Columbia College of Nurses and Midwives (BCCNM)
- Current First Aid and CPR certifications
- Computer literacy
- Strong desire to positively impact the lives of seniors by leading and mentoring care team members and coordinating care and services around the needs, abilities and preferences of seniors and their families
- Ability to communicate effectively both verbally and in writing
- Ability to deal with others effectively and work as part of a team
- Ability to organize and prioritize work
- Ability to operate related equipment
- Physical ability to carry out the duties of the position
- Skills and enhanced qualifications related to the professional nursing needs of special population groups as required
As this role is within an Assisted Living Facility environment, the successful candidate must also:
- Provide a clear Criminal Record Check
- Provide proof of Flu vaccinations and TB testing
- Provide proof of COVID-19 vaccination
- Hold a valid Food Safe Certificate
- Hold a valid WHMIS Certificate
- Follow all workplace health, safety and infection control policies
- Demonstrate professionalism and sensitivity when working with seniors and families
Required Documents
- Completion of application form with work history required
- Two character references with authorization for the employer to conduct reference checks
- Copies of diplomas, certificates and other relevant documents required
- Must be legally eligible to work in Canada
