WHAT YOU’LL DO:
This MSW position is a specialized role within the P.A.S.E. program, supporting the design the of the program, development, implementation, and evaluation of a new geriatric mental health group service for adults 65 years and older across four counties. The primary focus of this role is group development and facilitation, including the design and delivery of evidence‑based virtual and in‑person psychoeducation and psychotherapy groups for older adults. The Social Worker independently leads group intake and assessment, co-facilitates therapy sessions, monitors individual progress using a client‑centred approach, addresses group dynamics and conflict, provides crisis intervention as needed, and collaborates closely with P.A.S.E. clinicians and physicians on treatment planning. The role also includes developing group resources and educational materials and leading program evaluation activities to measure outcomes and inform service improvement.
As a secondary component, the Social Worker functions as a P.A.S.E. clinician as needed, providing comprehensive geriatric mental health assessments within an interdisciplinary team mandated to serve older adults. This includes education, consultation, and liaison with clients, caregivers, families, long‑term care homes, Behavioural Supports Ontario teams, and community partners. For a limited number of clients, the Social Worker supports continuity of care over time and across settings through individualized assessment, care planning, service coordination, monitoring, advocacy, and the provision of non‑pharmacological recommendations while individuals await geriatric psychiatry consultation. This role requires a clinician with strong experience working with older adults, demonstrated group work and program development expertise, and the ability to work independently, while maintaining professional standards and adhering to P.A.S.E. program requirements under the supervision of the Manager.
WHAT YOU’LL BRING:

Master’s degree in Social Work from an accredited program

Registered and in good standing with the Ontario College of Social Workers and Social Service Workers

Minimum three (3) years’ experience developing and facilitating therapeutic or psychiatric groups for older adults
-
Demonstrated expertise in group dynamics, effective group facilitation, and adult learning principles
-
Minimum three (3) years’ experience providing geriatric mental health assessment and intervention, including outreach with the psychogeriatric population
-
Experience leading the development, implementation, and evaluation of programs for older adults, with applied research expertise considered an asset

Strong assessment skills, including screening for depression, psychosis, suicide risk, and cognitive status using appropriate assessment tools

Proficiency in evidence‑based interventions for older adults, including CBT, DBT, trauma‑informed care, solution‑focused brief therapy, motivational interviewing, and brief action planning

PIECES trained

GPA training is considered an asset
WHO YOU ARE:

Demonstrated expertise in psychiatric and neurocognitive disorders in older adults, with strong assessment skills, including screening for depression, psychosis, suicide risk, and cognitive status, and the ability to formulate comprehensive psychosocial assessments

Extensive experience working with older adults, including the development of non‑pharmacological recommendations and the appropriate use of community resources

Strong grounding in social work theory and therapeutic modalities, including crisis intervention, short‑term counselling, family systems, case management, and the impact of serious mental illness

Demonstrated skill in group development, facilitation, and management

Strong experience in developing, implementing, and evaluating programs that serve older adults; applied research expertise considered an asset

Ability to support program evaluation, contribute to outcome measurement, and assist in identifying program priorities

Demonstrated ability to establish and maintain therapeutic relationships with clients and families over time

Knowledge of and ability to work collaboratively with long‑term care homes, hospitals, and community‑based services

Strong advocacy, critical thinking, problem‑solving, and consultation skills, including the provision of education using the PIECES framework

Superior communication skills (verbal, written, and interpersonal) and the ability to adapt messaging to diverse audiences

Demonstrated ability to work independently, manage a caseload, prioritize workload, and function effectively within an interdisciplinary team

Proficiency in Microsoft Office applications (Word, PowerPoint, Excel, Access) and electronic communication platforms

Valid driver’s license, access to a reliable vehicle, and willingness to travel across all four counties
