JOB SUMMARY:
Reporting to the RN/DOC/Designate, implements and evaluates nursing care to an assigned group of residents,to promote quality of life in accordance with the established policies and procedures of Finnish Home, standards of professional practice as outlined by the BCCNM.
DUTIES AND RESPONSIBILITIES:
- Provides direct resident care using the nursing process, which includes assessment, nursing diagnosis, care planning, implementation and evaluation.
- Administers medications (oral, topical, S/C, and IM) to assigned residents; documents their administration, monitors the resident reactions and evaluates their efficacy. Performs narcotic counts using two-person identification, places orders to refill medication carts and contingency supply.
- Performs nursing procedures such as wound care using sterile and non-sterile technique, colostomy care, tube feeding, urinary catheter insertion and irrigation, catheterization, vital signes and administering bowel care. .
- Completes RAI-MDS assessments for assigned resident
- Participates as a member of the interdisciplinary team by taking part in the development, implementation, assessment and evaluation of resident care plans and ADL’s.
Participates in preparation of care conferences
- Documents and updates information regarding the resident’s condition. and provides emergency response care.
- Assists residents with care as needed to meet activities of daily living such as continence care and toileting, personal hygiene, dressing and oral care. .
- Transfers , ambulates, and porters residents utilizing mechanical aides such as lifts,
- wheelchairs, and walkers as outlined in the residents care plan.
- Assists or cues the residents to eat and take adequate fluids as indicated by the residents individualized care plan and as directed by the dietitian.
- Completes incident reports for accidents and unusual incidents as per facility policies and procedures.
- Performs clerical functions that support resident care such as filing relevant documents in resident charts, faxing doctor’s orders to pharmacy, answering the phone and responding to issues.
- Participates in creating and maintaining a safe and healthy environment by understanding and following workplace health and safety rules, CCALA regulations and emergency management practices and infection control.
- Assists with the orientation of new LPN staff..
- Performs other related duties.
QUALIFICATIONS:
- Graduate of a recognized licensed practical nurse program or an equivalent combination of education, training, and experience.
- Current full practicing licensure with the BCCNM
- Ability to communicate effectively in English both verbally and in writing.
- Ability to lead others fairly and democratically.and collaborate with others.
- Physical ability to carry out the duties of the position
- Ability to organize work and manage time efficiently
Pay: $32.84-$41.35 per hour
