Job Summary
Comfort Keepers Ottawa is seeking a compassionate, self-motivated, and proactive Client Coordinator to join our team. This role is essential in ensuring exceptional service delivery by coordinating care for our clients, supporting caregivers, and maintaining strong relationships with clients, families, and healthcare professionals.
Duties
- Serve as the primary point of contact for clients and their families, providing timely, professional, and compassionate support throughout the client journey.
- Develop, implement, and maintain individualized care plans tailored to each client's unique needs, preferences, goals, and changing circumstances, ensuring services are delivered safely, consistently, and in accordance with the established plan of care.
- Conduct regular client follow-ups to monitor satisfaction, address concerns, and identify opportunities to enhance the client experience.
- Analyze client feedback and service data to identify opportunities for continuous improvement and client retention.
- Prepare reports on client activity, service quality, and key performance indicators for internal review.
- Facilitate effective communication between clients, caregivers, and internal team members to ensure seamless service delivery.
- Support the onboarding of new clients by coordinating assessments, explaining services, and ensuring a smooth transition into care.
- Collaborate with the scheduling team to support continuity of care and timely service adjustments as client needs evolve.
Experience
- Diploma or Bachelor's degree in Healthcare, Social Work, Business Administration, or a related field is preferred.
- Minimum of 1–2 years of experience in an administrative, scheduling, client coordination, or care coordination role within a healthcare, home care, or social services environment.
- Proficiency in Microsoft Office (Word, Excel, Outlook) and experience using AxisCare or similar home care management software for scheduling, client records, caregiver coordination, and data management.
- Excellent written and verbal communication skills, with the ability to build positive relationships with clients, families, caregivers, and healthcare professionals.
- Strong organizational, time management, and problem-solving skills, with the ability to prioritize multiple tasks.
- Exceptional attention to detail and a commitment to accuracy and confidentiality.
- Demonstrated ability to handle sensitive situations with professionalism, empathy, and sound judgment.
- Self-motivated and proactive, with the ability to take initiative, work independently, and collaborate effectively with colleagues to deliver exceptional client care and service.

Position: Part-Time (with the opportunity to transition to Full-Time based on performance and business needs)
