Job Summary
We are seeking a dedicated RN and RPN to provide compassionate and comprehensive nursing care in a long-term care setting in Cornwall . The ideal candidate will have experience in nursing home environments and a strong background in patient-centered care, including dementia, senior care, and hospice services. This role involves collaborating with interdisciplinary teams to ensure the well-being and safety of residents while maintaining high standards of clinical practice.
Duties
- Assist with activities of daily living, including hygiene, mobility, and nutrition support
- Document patient care activities thoroughly using electronic health records such as Epic or Cerner systems
- Collaborate with physicians, therapists, and family members to develop and update individualized care plans
- Provide specialized care for residents with dementia, hospice needs, or other chronic conditions
- Educate residents and families about health conditions, medication management, and discharge instructions
- Ensure compliance with healthcare regulations and safety standards within the facility
Qualifications
- Successful completion of a recognized RN and RPN diploma from an approved nursing program (Ontario or equivalent jurisdiction)
- Current registration or eligibility for registration with the College of Nurses of Ontario (CNO)
- Previous clinical experience in long-term care, hospital, or nursing home settings (preferred or required depending on employer)
- Experience working with diverse populations, including seniors, palliative, or hospice care patients
- Clear Vulnerable Sector Check (VSC) issued within the last 6 months
- Current Tuberculosis (TB) screening Step 1 and Step 2 completed within the last 6 months
- Up-to-date immunization records, including MMR (Measles, Mumps, Rubella)
- Current CPR Certification (Basic Life Support – BLS level or equivalent)
