Position Summary
The Support Worker provides direct personal care and daily living supports to individuals residing in staffed residential homes. Working as part of a multidisciplinary team, the Support Worker assists individuals with activities of daily living while promoting their health, safety, well-being, and independence in accordance with individualized support plans.
Responsibilities
- Provide personal care and assistance with activities of daily living, including bathing, dressing, grooming, toileting, and mobility.
- Assist individuals with meals, hydration, and feeding as required.
- Monitor and report changes in an individual’s physical, emotional, or behavioural condition.
- Measure and record vital signs such as blood pressure, temperature, and pulse, when required and trained to do so.
- Assist with medication administration in accordance with agency policies, training, and delegated responsibilities.
- Follow individualized support plans, behaviour support plans, and health care protocols.
- Maintain accurate documentation and complete daily records, medication records, and incident reports.
- Support individuals to maintain clean, safe, and organized living environments, including making beds, laundry, housekeeping, and inventory of household and personal care supplies.
- Assist individuals with prescribed exercise programs, mobility, and therapeutic activities as directed.
- Respond promptly to requests for assistance and ensure the comfort and well-being of individuals receiving support.
- Collect specimens when required and trained to do so.
- Transport and accompany individuals to appointments and community activities using agency vehicles or mobility equipment.
- Promote dignity, independence, choice, and inclusion while providing person-centred supports.
- Communicate effectively with supervisors, health care professionals, families, and other members of the interdisciplinary team.
- Perform other duties related to personal care, health support, comfort, and residential services.
- Support individuals to develop daily living skills, participate in household responsibilities and community activities, make informed choices and make informed choices, and work toward goals identified in their individualized support plans.

Pay: $24.74-$25.87 per hour
