Position: Manager, Specialized Care and Programs

Position Summary:
The Manager, Specialized Care and Programs provides operational leadership, coordination, and oversight across multiple clinical programs within the Couchiching Family Health Team (CFHT), including the Unattached Care Clinic, Palliative Nurse Navigator Program, Supported Attachment Team, and Primary Care Expansion Team (Nurse Practitioners and Registered Practical Nurses).
Reporting to the Director, Primary Care Attachment and Community Partnerships, the Manager ensures the delivery of high-quality, patient-centred care across all assigned programs while fostering a collaborative, accountable, and values-driven culture. The role also acts as a key advisor to senior leadership in support of the CFHT Strategic Plan, aligned with Ontario Health Team (OHT) priorities and broader provincial health system direction.
The Manager collaborates with internal and external partners to strengthen integrated care pathways, improve access to services, and support system navigation for unattached and complex patient populations.

Roles and Responsibilities
- Provide leadership and operational oversight across all assigned programs, ensuring effective coordination, integration, and service delivery
- Advise and support senior leadership in advancing organizational priorities and the CFHT Strategic Plan and DEI Initiative
- Provide day-to-day leadership to clinical and administrative staff, promoting a positive, respectful, and values-based work environment
- Support staff engagement, wellness, retention, and professional development through coaching, stay interviews, and performance management
- Supervise direct reports, including scheduling, workload coordination, and approval of time off requests
- Conduct performance reviews every 2 years, provide ongoing feedback, and support employee development planning
- Ensure patient safety issues are identified, addressed, and escalated appropriately
- Support resolution of patient concerns and complaints in collaboration with the Director of Clinical Services
- Lead onboarding, orientation, and offboarding processes for staff within assigned programs
- Promote collaboration and manage conflict effectively within and across teams
- Support policy implementation related to HR processes, onboarding/offboarding, and operational workflows
- Participate in and/or chair internal meetings as required, including All Team Meetings and program-specific meetings
- Ensure accurate data collection, reporting, and submission for program performance and accountability purposes
- Complete chart audits and quality improvement activities in collaboration with senior leadership
- Support education, training, and mentorship of allied health professionals and learners
- Review and coordinate student placements and volunteer requests in collaboration with HR
- Maintain compliance with health and safety legislation and organizational policies
- Promote awareness of the FHT services and programs to the patients of the FHT
- Ensure data is collected and reviewed for accuracy for statistical/reporting purposes and ensure they are handed in on time to the Director of Clinical Services
- Lead and offer clinical guidance and education to allied health professionals ensuring job satisfaction and promote learning opportunities
- Provide financial oversight, including budget development, expenditure monitoring, variance analysis, and alignment with funding and service agreements
- Perform other duties as required
Skills/Knowledge Requirements:
- University degree in a regulated Health Care Profession (I.e., Nursing) and/or master’s degree in a Regulated Health Care Profession (I.e., Health Administration, Health Management), or equivalent combination of education and experience
- Certificate of registration with a Regulated Health Care Professional Association
- Demonstrated leadership experience in healthcare or community health settings
- Strong operational management skills across multiple programs or service areas
- Excellent interpersonal, communication, and conflict resolution skills
- Proven ability in staff supervision, coaching, and performance management
- Strong organizational, analytical, and decision-making skills
- Experience working within integrated care models considered an asset
- Ability to work effectively in a complex, multi-stakeholder environment
- Strong cultural competency and commitment to equitable, patient-centred care
- Proficiency with electronic systems, reporting tools, and administrative platforms
- Works in compliance with Health and Safety laws adhering to safe work practices and procedures established by the CFHT
- Provide a Police Record/Vulnerable Sector Check.

Pay: $78,000.00-$82,000.00 per year
