The Nogojiwanong Friendship Centre (NFC) is a not-for-profit, culture and community-based organization mandated to serve the needs of urban Indigenous people in the City of Peterborough and surrounding area. The NFC is currently accepting applications for a full-time position as our Life Long Care (LLC) Program Worker

Objectives:
To support urban Indigenous Elders, seniors and/or people living with chronic illness and disability to live independently, staying in their communities, and achieving and maintaining health and well-being
Responsibilities:
The Life Long Care (LLC) Program Worker will work to improve the quality of life for clients by providing ongoing mental, physical, emotional, and spiritual support. Additionally, will work closely within the LLC team to ensure the LLC program is effectively planned, implemented, maintained, and evaluated. In addition, will demonstrate cultural awareness and safety while using a trauma-informed, holistic approach in the community.
The LLC worker will:
- Provide individuals with one-to-one and participant-based support services;
- Develop support services based on community-identified needs such as transportation, caregiver support, socialization opportunities, home visiting, assistance with food insecurity, home maintenance and repairs, service navigation, and help with non-insured health benefits programs.
- Provide access to long-term care programs, services and cultural/spiritual supports;
- Develop and arrange informative, educational sessions and life skills training related to health and wellness;
- Navigate Non-Insured Health Benefits (NIHB) and provide training and support to other staff related to NIHB
- Ensure program access through a variety of outreach efforts and promotional activities within the community;
- Facilitate a strong community support system via respite care to ensure families are supported
- Develop a comprehensive volunteer support circle;
Qualifications:
- Post-secondary education in Indigenous studies, social work, or related field.
- Minimum 3 years of demonstrated and related work experience in health or social services.
- Other combinations of education and experience will be considered
- Strong knowledge and awareness of Indigenous culture and history.
- Knowledge of local and regional health, wellness and long-term care support service agencies, and Non-Insured Health Benefits.
- High proficiency in Microsoft 365 (Word, Excel, PowerPoint, etc.).
- Outstanding communication, analytical, problem-solving, and troubleshooting skills.
- Ability to work under pressure, prioritize work and meet tight deadlines.
- Ability to work independently and as a part of a team, with minimal supervision
- Valid driver’s license, a reliable vehicle and be willing to travel when required;
- First Aid / CPR certification or be willing to be certified;
- A satisfactory Criminal Reference Check (CPIC) with a Vulnerable Sector Search (VSS) will be a condition of the employment offer.
Salary: $45,900.00 - $60,500.00 (with a 3-month probation)
Benefits: Health care spending account, 2 weeks paid vacation, 10 days of paid sick time, 5 paid wellness days, holiday closures.
Schedule: 8:30 - 4:30 Monday to Friday with evenings and weekends as needed
