The successful candidate will serve as an advocate and champion for our patients and will be compassionate and caring with a focus on patient and family-centred care. Ability to prioritize and function within a team is crucial. Proven track record for maintaining professional boundaries and practicing within the established policy and procedures of Assisted Living Alberta. The Social Worker II works within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships. In addition, the Social Worker is responsible for the provision of social work services to clients and families including psychosocial assessment, crisis intervention, education, advocacy, resource finding, consultation, and making appropriate referrals to members of the multidisciplinary team.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:
Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP). Knowledge and experience working with diverse and marginalized populations using an equity, diversity, inclusion lens. Knowledge and application of trauma informed care. Understanding of the impact of Indigenous people’s history, culture and of reconciliation.
Preferred Qualifications:
Previous experience working in an acute care and rural setting an asset. Excellent communication skills, written and verbal. Experience working within an interdisciplinary team environment. Knowledge of and experience with guardianship, trusteeship and capacity assessment processes. In-depth knowledge and experience in working with Income Security programs at community/provincial/federal levels (Alberta Works, AISH, ASB, CPP). Experience working with Connect Care an asset.
