Why join the team?
You will play a vital role as part of a multidisciplinary team that assists older adults with their acute health care needs, including providing care to those who have experienced significant functional decline in a short time frame, or who are experiencing multiple complex medical and/or psychiatric problems. In addition to your clinical responsibilities, you will provide professional guidance to other healthcare professionals, working autonomously and collaboratively within an interdisciplinary healthcare team. Your role will involve making referrals to specialist physicians and other resources as necessary. You will collaborate with patients, clients, residents, and healthcare professionals to identify trends and patterns that impact the community's health and participate in evidence-based strategies to improve healthcare services. Your involvement in policy-making activities will also influence healthcare practices.
