The Family Practice Nurse (FPN) is a Licensed Practical Nurse who will work collaboratively within a family practice environment to enhance the delivery of primary care services in alignment with the Patient’s Medical Home Model of Care.
The FPN will work to full scope of practice within the Grande Prairie Primary Care Network (GP PCN) Fairview member clinic. The FPN will provide a broad range of services, including but not limited to, health education and promotion, management of routine screening maneuvers, chronic disease prevention and management, acute episodic care, therapeutic interventions, well-woman/well-baby care, and care coordination. The FPN will be a complementary source of support to the physician providing wound care, injections, and other clinical interventions to patients as directed by the physician. The FPN will have scheduled patient appointments and will be available as needed for non-scheduled patient encounters.
Duties & Responsibilities:
General:
· Facilitate continuity of care and support comprehensive team-based care under the supervision and leadership of the primary care physician(s)
· Coordinate and manage patients’ care within professional scope of practice
· Utilize a patient and family-partnered care approach to care delivery which ensures that patients/families are partners in their health care planning and treatment
· Collaborate and communicate effectively with primary care physicians, PCN team members, health care professionals from external agencies, and contacts at other health/social care organizations
· Integrate best practices in the maintenance and management of patient panel(s)
· Organize and standardize the Electronic Medical Record (EMR) software for optimal use
· Maintain professional requirements, current knowledge and skills and collaborate with the physician(s), PCN Transformation Manager and PCN Nurse Liaison to identify professional development and training needs
· Assist in reviewing and providing feedback for developing clinical policies and protocols
· Utilize quality improvement activities to improve care delivery
· Participate in evaluation and reporting activities related to the clinical role and the PCN’s Practice Transformation Program (PTP)
· Attend relevant PCN education and training opportunities, interdisciplinary team meetings, and nursing meetings as required
Clinical:
· The FPN will have scheduled patient appointments and be available for non-scheduled patient encounters.
· Provide education and counselling on chronic diseases, preventative care, and other health conditions.
· Offer wound care, injections, ear syringing, fetal heart monitoring and other clinical skills as required and directed by physician.
· Routinely review chronic disease registries to ensure patients are completing routine screening and managing health conditions.
· Provide routine screening measures (blood pressure, foot screens, etc.) and refer to other health care professionals as required (ophthalmologist for eye exams.)
· Identify patients who are due for preventative screening measures (Alberta Screening and Prevention). Work collaboratively with physician to provide requisitions and follow up for same.
· Provide routine follow up to patients after medication or lifestyle changes.
· Assist with supporting documents for patients (AISH, disability or insurance paperwork, medicals)
· Coordinate referrals to PCN programs and community agencies.
· Role expansion opportunities (i.e., paps)

Required Skills & Abilities:
· Fluency in English language (written and verbal)
· Computer skills: email, MS Word, MS Excel, EMR software
· Effective teaching and communication skills
· Knowledge of Nursing Competency Profile and Standards of Practice
· Ability to recognize change in a patient’s health status and act accordingly
· Ability to maintain accurate records in line with practice policy
· Ability to make evidence-based decisions in accordance with best practice guidelines
· Knowledge of privacy legislation and associated obligations within a health care setting
· Ability to build collaborative and respectful relationships with patients, families, and other healthcare professionals
· Awareness of the Patient’s Medical Home Model of Care, social determinants of health, Alberta Screening and Prevention maneuvers (ASaP) and principles of self-management
· Ability to occasionally work outside of normal business hours
Use of vehicle for business use will be required which necessitates possession of a valid Alberta driver’s license and access to a motor-vehicle insured for work-related travel
Required Qualifications:
· Registered with the College of Licensed Practical Nurses of Alberta and in good standing
· Current cardiopulmonary resuscitation (CPR) certification – BLS for Health Care Providers
· Current Criminal Record Check and Vulnerable Sector Check
· Diploma in Practical Nursing
· Combined minimum of 3 years of primary care, acute care and chronic disease management nursing experience preferred however new grad’s are welcome to apply.
· Experience working in a care team preferred
· Certifications in wound care, diabetes education/foot care are considered an asset
