The East Calgary Family Care Clinic is known for its innovative approach to providing comprehensive primary care for people with complex needs. This could be any combination of chronic illnesses, mental health challenges and addiction along with social, economic or cultural factors that negatively impact health. Reporting to the Program Manager, the Care Manager provides supervisory support to staff working at the FCC in addition to non-reporting staff: clinical pharmacist and dietitians. The Care Manager works to provide clinical support, coaching and leadership to staff in accordance with the Primary Care & Chronic Disease Prevention & Management portfolio within Primary Care Alberta. The Care Manager develops and maintains strong working relationships with key health agency stakeholders (ACA, ALA, RA), community organizations, not for profit organizations, and PCNs in collaboration with the Program Manager and FCC Medical Director. The position is responsible and accountable for clinic level decision making and provides direct leadership in achieving clinic goals and objectives.
The Care Manager is responsible for, or in conjunction with the leadership team, the following: Manages human, financial, and physical resources to support the effective and efficient delivery of patient care and clinic operations. Provides leadership for regulated and non-regulated healthcare staff through effective communication, shared problem solving, coaching, supporting professional development, and performance management. Works within a framework of collaborative, interprofessional practice, and team-based care. Operationalizes standards of practices, internal processes, & policies. Leads quality improvement initiatives by identifying key challenges and opportunities, support implementation of innovative approaches and fostering a culture of continuous improvement focused on patient and family centered care. Commits to continually improving health and safety performance through WHS processes, RLS reports, and incident investigations and/or management. Contributes to the development and implementation of the portfolio strategic plan and the establishment of operational priorities for the East Calgary Family Care Clinic.

Required Qualifications:
A minimum of a Baccalaureate degree in Nursing and/or a health-related degree. Active registration with the applicable College or licensing body. A minimum of 3 years' experience in health care, preferably in a community care setting with related clinical experience and knowledge. A minimum of 3 additional years in a leadership position with demonstrated excellent interpersonal and communication skills. Demonstrated clear pattern of professional and personal development with leadership abilities and a strong commitment to operating within team environments. Demonstrated change management & conflict resolution skills. Demonstrative ability to establish strong partnerships and collaborative working relationships with internal and external stakeholders to achieve organizational objectives. Self-directed and highly motivated. Demonstrated competency in managing multiple priorities. Experience working with and leading groups providing care to equity deserving and/or diverse groups.
