PURPOSE
Working within an interdisciplinary Diabetes Education Program (DEP) team, the Registered Dietitian provides comprehensive diabetes prevention and management support for individuals living with type 2 diabetes and pre-diabetes. Using a client-centered, self-management approach, the RD delivers evidence-based nutritional assessment, counseling, education, and health promotion services that empower clients to improve diabetes outcomes and reduce complications.
The role supports WellFort’s commitment to equitable access to chronic disease management services by delivering culturally responsive care in both clinic and community settings throughout Brampton and Malton. The position integrates direct client care, diabetes education, community outreach, advocacy, and collaborative program development to address the social determinants impacting diabetes prevention and self-management.
RESPONSIBILITIES
- Co-facilitate Diabetes Education Program (DEP) group sessions for adults living with type 2 diabetes and pre-diabetes using evidence-based nutrition and diabetes self-management education.
- Provide individualized nutrition assessments and counseling that consider medical, social, cultural, economic, and psychosocial factors affecting diabetes management.
- Support clients in setting realistic, client-driven health goals using motivational interviewing and self-management strategies.
- Deliver diabetes prevention and chronic disease education through individual counseling, group facilitation, and community outreach initiatives.
- Adapt teaching methods and educational materials to meet diverse literacy levels, cultural backgrounds, and client learning needs.
- Promote equitable and barrier-free access to diabetes care for priority populations experiencing health inequities.
- Collaborate with primary care providers, interdisciplinary teams, and community partners to ensure continuity of diabetes care and coordinated client support.
- Participate in community-based health promotion initiatives focused on diabetes prevention, nutrition education, and chronic disease awareness.
- Identify emerging trends and barriers affecting diabetes outcomes within priority populations to support advocacy, program planning, and service improvement.
- Maintain accurate and timely client documentation in accordance with clinical and organizational standards.
- Provide services across WellFort office locations and community sites based on program and client needs.
QUALIFICATIONS, EXPERIENCE & SKILLS
- Undergraduate degree in food and nutrition from a recognized university or equivalent.
- Completion of a Canadian Dietetic Association accredited Dietetic Internship
- Registered with the College of Dietitians of Ontario in good standing
- A minimum two years experience working as a diabetes educator or working in a community-based health promotion setting working in a similar position.
- Certified Diabetes Educator (CDE) is an asset or willingness to acquire.
- Demonstrated experience in health promotion program planning, development, implementation, and evaluation.
- Demonstrated experience in facilitating groups and providing individual counseling using a client centered approach.
- Demonstrated commitment to best practices, ongoing learning, and client centered care.
- Experience working with communities and individuals who face barriers accessing traditional health care institutions due to poverty, discrimination, mental health and substance use issues.
- An understanding of and critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health.
- Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level.
- Experience working with confidential and time sensitive documents and information.
- Excellent interpersonal skills and the ability to multitask in a fast paced environment.
- Experience working in a value based non-profit or social services organization.
- Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.
- Demonstrated proficiency in use of computers and various computer software applications, such as Electronic Medical Record (EMR).
- The ability to speak languages relevant to the residents of the organization’s community is an asset.
- Holds a valid Ontario driver’s license, and access to a reliable vehicle with insurance for responsive care delivery at any care delivery location.
