We are currently seeking a Service Resolution Coordinator to join our dynamic team.

Hours of Work: 35 hours a week
Responsibilities:
- Work collaboratively with community service agencies to support the Service Resolution process and the function of the Children’s Review Committee (CRC) Peel.
- Strengthen the capacity of the Service Resolution Mechanism to continue to creatively and effectively respond to children and youth in Peel with complex needs.
- Review service requests to ensure referral documentation is accurate and compliant with established processes.
- Serve as key resource and point of contact for service agencies, service coordinators, senior leaders and community partners.
- Coordinate referrals, presentations and related activities for the Children’s Review Committee.
- Respond to consultations, inquiries and requests for case conferences in a timely manner.
- Build upon existing service coordination practices and collaborative efforts, including participation in case conferences, developing working relationships and maintaining contact with community agencies, cross-sector partners and Ministry representatives to enhance service coordination and resource sharing.
- Maintain current knowledge of community resources both within and outside Peel Region.
- Facilitate out of region referrals assisting case managers and service coordinators with resource and referral information.
- Set up and support meetings, prepare and distribute agendas and minutes, disseminate relevant information to participants and recommendations to presenters.
- Monitor, analyse and report outcomes and issues, including gaps and trends related to the Service Resolution function in the form of gathering and maintaining statistical data, annual reviews and reports.
- Participate in working groups, task groups, committees and advisory tables to support system planning and improve community response to complex needs.
- Work flexible hours.
Qualifications and Experience:
- Undergraduate degree in Social Services or related education, or an equivalent combination of education and work experience in the sector
- An understanding of Service Resolution system and issues facing children with complex needs in the Region of Peel
- Experience in program development, implementation, evaluation, and statistical data reporting
- Experience with service navigation and case conference protocols within the developmental and mental health sectors
- Experience managing a budget
- Comfortable presenting in front of a professional audience
- Strong engagement, organizational, communication and creative thinking skills
- Ability to work independently and as a team player, in dynamic settings
- Fluency in French or other languages is considered an asset
Salary Range: $71,968 - $81,331
