Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively with diverse patient populations, including seniors, children, and individuals with complex medical needs. The ideal candidate will be committed to delivering personalized care while adhering to all safety and regulatory standards.
Responsibilities
- Administer medications and treatments accurately following physician orders and established protocols
- Monitor patient vital signs and observe for any changes in condition
- Assist with patient assessments and document findings in electronic health records such as Epic or Cerner systems
- Support patients with activities of daily living, including hygiene, mobility, and nutrition
- Provide specialized care for patients with dementia, hospice needs, or acute medical conditions
- Perform phlebotomy and laboratory specimen collection as needed
- Educate patients and families about health management, medication use, and post-discharge care instructions
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Ensure compliance with infection control procedures and safety standards in all patient interactions
- Maintain accurate documentation of nursing interventions and patient responses
Experience
- Valid LPN license in the state of practice
- Prior experience in nursing homes, hospitals, or home care settings preferred
- Experience in acute care, dementia care, pediatrics, senior care, hospice, or assisted living environments is highly desirable
- Knowledge of medical terminology and medication administration protocols
- Familiarity with electronic health record systems such as Epic or Cerner is a plus
- Additional skills may include phlebotomy, laboratory procedures, pain management techniques, case management experience, and working with diverse patient populations including children and seniors
