Our team works collaboratively to empower individuals to reach their personal goals through respect, choice, and meaningful community involvement.Position SummaryThe Disability Community Support Worker provides compassionate, person-centered support to adults with disabilities in both residential and community-based settings. This role is dedicated to enhancing individuals’ independence, quality of life, safety, and overall well-being through respectful, individualized care and support services.Key Responsibilities
- Assist individuals with activities of daily living, including personal hygiene, grooming, dressing, and meal preparation.
- Administer or support medication management in accordance with established care plans, policies, and procedures.
- Foster meaningful relationships by providing emotional support, companionship, and positive encouragement.
- Assist with mobility, transfers, and lifting while ensuring the safety, comfort, and dignity of individuals receiving support.
- Facilitate community engagement and participation through social, recreational, and community-based activities.
- Monitor, document, and promptly report changes in health status, behaviour, or support requirements.
- Maintain a clean, safe, and organized living environment, including performing light housekeeping duties as required.
- Complete accurate and timely documentation of daily activities, progress notes, incidents, and support outcomes.
- Collaborate effectively with interdisciplinary team members, family members, and supervisors to ensure the delivery of high-quality, person-centered care.
Qualifications & Requirements
- Experience in disability support, community care, or home support (preferred)
- Knowledge of person-centred and respectful support practices
- Ability to assist with personal care, mobility, and lifting
- Strong communication, teamwork, and interpersonal skills
- A compassionate, patient, and reliable approach to care
- Clear Vulnerable Sector Police Check (completed within the last 6 months)
- Valid First Aid & CPR Certification (Basic or Standard )
- Legal entitlement to work in Canada (valid work or study permit, if applicable)
- Flexible availability, including evenings and weekends
- Valid Class 5 driver’s license, reliable vehicle, and $2M liability insurance
