Career Opportunity
We are looking to hire two Service Coordinators , full time and full time contract, to join our Halton Support Services team! In this position, you will play an important role in ensuring the provision of appropriate social and community support services to maximize the potential for independence for people with developmental disabilities living in community settings or with their families, This role will assist them in strengthening their capacity to manage and acquire the skills that will help them enhance their support network, awareness of generic community-based resources, government-funded services and supports.
RESPONSIBILITIES:
- Providing social and community support services to individuals with developmental disabilities living in community settings or with their families.
- Provides services through the following programs: Adult Program, Children’s Program and Respite Program.
- Providing service coordination through a process that include: accessing resources to meet individuals’ specific needs, working with the individual to develop goals/objectives, coordinating community resources to meet individuals’ specific needs, assisting the individual to utilize community resources, monitoring and evaluating resources’ effectiveness upon goals.
- Supporting individuals through short- or long-term planning processes, including during periods of crisis, and making referrals to appropriate community agencies as needed. Assisting and supporting families according to an individual’s age and social situation.
- Providing help and instruction with problem solving around daily living skills.
- Providing alternative, complementary models of receiving support.
- Acting on behalf of the individual in relationship to community resources.
- Facilitating problem solving and resolution with service providers.
- Carrying out administrative functions that supplement the direct work with individuals.
- Maintaining an awareness of all legislation from all levels of government that affects individuals.
- Participating in activities providing public education, outreach and the ongoing development of the service delivery system.
- Other duties as assigned.
WHAT QUALIFICATIONS AND SKILLS DO YOU BRING?
- Developmental Services Worker Diploma (DSW).
- Candidates who do not hold a DSW diploma may be considered if they have a Diploma (minimum 2-year program) or Degree from a recognized Community College/University in Social/Human Services.
- A minimum of two years’ previous experience supporting people with a developmental disability, mental illness or Dual Diagnosis.
- Previous service coordination experience is a requirement.
- Good knowledge of the social services resources and the professional service providers within the greater community area.
- Ability to coordinate, facilitate and provide training to community service providers.
- Effective assessment, planning, priority setting and problem-solving skills.
- Good oral/written communication skills.
- Proficiency in Microsoft Office including Word and Excel.
- Ability and willingness to work flexible hours to meet with the clients and community resources.
- An ability to work on a hybrid model, which includes in-office/ in the community and work from home, per organizational needs.
- Willingness to drive across the municipalities supported by CWSDS within Halton Region.
- Valid Ontario G class driver’s license and vehicle for conducting visits and outreach activities.
- Providing proof of vaccination for COVID-19 and maintaining vaccination status as necessary from time to time during employment.
