Alderwood is a 70 bed Nursing Home/Residential Care Facility. Our building has 6 separate home areas: five home areas for Nursing Home (11 or 12 beds) and one home area for Residential Care (11 beds). All are private rooms with private bathrooms.

Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our healthcare team. The ideal candidate will play a vital role in managing patient care plans, coordinating services, and ensuring seamless communication between patients, families, and healthcare providers. The Care Coordinator will provide leadership, guidance and teaching to other roles including CCAs and LPNs. The Care Coordinator will utilize their clinical knowledge and case management skills to support residents and make a meaningful difference in resident outcomes.
Responsibilities
- Organize daily responsibilities for the Nursing Department by arranging resident appointments for blood work, treatments, priorities and instruction for any changes in general care, routine physician responsibilities, processing orders, etc.
- Make referrals for Physiotherapy, Occupational Therapy, Palliative Care, etc. in collaboration with the Nursing Dept and follow through on recommendations.
- Develop and implement individualized care plans tailored to each residents needs.
- Coordinate medical services across multiple providers and departments to ensure continuity of care.
- Serve as a primary point of contact for patients and their families, providing education and emotional support.
- Monitor resident progress and review and update Care Plans and Kardexes with the Nursing team during conference and communicate changes with staff.
- Assist with quarterly med reviews.
- Document all interactions accurately within electronic health records while maintaining confidentiality.
- Stay informed about current healthcare regulations, policies, and best practices related to case management and patient care. Experience
- Demonstrated experience in case management or patient advocacy is desirable.
- This role requires a compassionate approach combined with strong organizational skills to effectively coordinate complex care needs. The successful candidate will be committed to improving resident outcomes through comprehensive case management and collaborative teamwork.
Pay Range is as per the NSNU Collective Agreement
Pay: $38.01-$48.50 per hour
