Summary
The Case Manager of Community Connections delivers trauma-informed, culturally grounded, and person-centered care coordination for urban Indigenous community members, families, and individuals at various stages of their life journey.
Housed within the Community Connections team, this versatile role serves as both a direct entry point for community members navigating general systems, life skills, and cultural connections, as well as a transition bridge for individuals stepping down from higher-intensity housing or justice programs. By offering flexible support tailored to individual needs—ranging from short-term navigation to intensive, long-term care coordination—this role fosters personal autonomy, emotional well-being, and community belonging while ensuring seamless service continuity across First Light’s core offerings.

You Bring:
- Strong relationship-building skills and the ability to establish trust with diverse community members

- Experience supporting individuals through complex systems including housing, healthcare, income support, justice, education, or employment
- Confidence navigating difficult conversations with professionalism and compassion
- Strong organizational skills and the ability to balance multiple priorities
- Clear communication, accurate documentation, and professional accountability
- An understanding of harm reduction, emotional regulation, and strengths-based practice
What You'll DoRelationship-Based Case Management
- Welcome community members through scheduled appointments and drop-in supports.
- Complete intakes and collaboratively develop individualized wellness plans.
- Provide ongoing case management that reflects each person's goals, strengths, and priorities.
- Support individuals transitioning from housing and justice programs as they reconnect with community.
- Coach community members in life skills, budgeting, systems navigation, emotional regulation, and self-advocacy.
Advocacy & Systems Navigation
- Help community members access housing, healthcare, income supports, education, employment, legal services, and other community resources.
- Advocate alongside individuals to reduce barriers and improve access to services.
- Coordinate care with internal programs and external partners to ensure seamless support.
- Respond to crises through de-escalation, safety planning, and timely referrals.
Culture & Community Connection
- Incorporate Indigenous ways of knowing, teachings, and cultural practices into daily work.
- Encourage participation in cultural programming, ceremonies, workshops, and community events.
- Foster welcoming and inclusive spaces where everyone feels respected, supported, and connected.
- Build relationships that strengthen belonging and long-term community wellness.
Collaboration & Accountability
- Work closely with interdisciplinary teams to provide coordinated wraparound care.
- Maintain accurate and confidential documentation.
- Track outcomes and contribute to program reporting and evaluation.
- Participate in clinical supervision, reflective practice, and ongoing learning.
- Contribute to continuous improvement while supporting the vision and values of First Light.
Skills, Experience & RequirementsWe recognize that knowledge comes through many paths—including formal education, community work, cultural teachings, and lived experience. We value people who bring compassion, humility, and a genuine commitment to walking alongside others.To be successful in this role, you'll have:
- A degree in Social Work, Nursing, Human Services, Psychology, Sociology, Indigenous Studies, or a related field; or an equivalent combination of education, community experience, and lived experience.
- Experience providing case management, care coordination, or community-based support.
- Knowledge of trauma-informed practice, harm reduction, and person-centred care.
- Experience working with Indigenous communities or a demonstrated commitment to culturally safe practice.
- Excellent communication, documentation, and relationship-building skills.

- Registration as a Social Worker (RSW)
- First Aid and CPR certification

- Crisis Intervention or De-escalation training
- Knowledge of anti-oppressive practice and systems navigation
- Valid driver's licence and access to a reliable vehicle
- Satisfactory Record of Conduct and Vulnerable Sector Check
- Flexibility to work occasional evenings or weekends to support programming and community events
Pay: $55,000.00-$65,000.00 per year
