CareVitas Home Care is looking for a compassionate, organized, and motivated Client Care Coordinator & Community Outreach team member to help support families and grow our home care services across Vaughan and the GTA.
This role includes answering inquiries, office support, lead follow-up, client care coordination, family communication, community outreach, occasional PSW/caregiver onboarding, and occasional field visits.
The right person should be comfortable speaking with families, following up with potential clients, booking care assessments, assisting with office/admin tasks, and visiting clients in their homes when needed.
CareVitas is a growing home care startup, so we are looking for someone flexible, self-motivated, and comfortable helping us build strong relationships from the ground up.
Responsibilities
Responsibilities include answering phone and email inquiries, following up with leads and potential clients, booking free in-home care assessments, assisting with office/admin tasks, supporting new client onboarding, and keeping records of client updates, leads, and follow-ups.
This role may also include checking in with existing clients, visiting clients when needed, helping resolve client or family concerns, and communicating updates between families, caregivers, office staff, and management.
This role also includes reaching out to referral sources and community partners, building strong relationships, and helping connect families with the right CareVitas Home Care services.
The role may also involve occasionally assisting with onboarding new PSWs/caregivers, including basic orientation, introducing CareVitas expectations, and helping ensure they understand client needs before starting care.
Ideal Candidate
We are looking for someone with experience in home care, healthcare, senior care, customer service, office administration, lead follow-up, community outreach, client coordination, caregiver coordination, or relationship building.
The ideal candidate should be professional, compassionate, organized, reliable, comfortable speaking with families, and able to travel within Vaughan and the GTA when needed. A vehicle is preferred.
Compensation
Compensation includes $18–$22 per hour, depending on experience, for approved office work, client visits, follow-ups, outreach, admin work, onboarding support, and client support duties.
The role also includes 8% commission on paid invoices for new clients personally signed up by the employee, payable for the first 30 days of service only.
Commission is paid only after client payment has been received. No commission is paid on cancelled, unpaid, refunded, or incomplete services.
