Overview
We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients. This position requires strong clinical skills, attention to detail, and a commitment to patient care. The Pharmacist will work closely with healthcare providers to optimize medication therapy and improve patient outcomes in both acute care and hospital settings.
Responsibilities
- Review and interpret physician orders for accuracy and appropriateness of medication therapy.
- Prepare and dispense medications in accordance with established protocols.
- Provide medication administration guidance and support to healthcare staff.
- Conduct patient consultations to ensure understanding of medication regimens, side effects, and interactions.
- Collaborate with multidisciplinary teams to develop individualized patient care plans.
- Maintain accurate records of medication dispensing and patient profiles.
- Monitor patient responses to medications, making adjustments as necessary under physician guidance.
- Stay updated on new medications, therapies, and medical terminology relevant to pediatrics and acute care.
Experience
- A Doctor of Pharmacy (Pharm.D.) degree from an accredited institution is required.
- Active pharmacist license in the state of practice.
- Previous experience in a hospital or acute care setting is preferred but not mandatory.
- Familiarity with pediatric medication administration is a plus.
- Strong understanding of medical terminology and its application in patient care.
- Excellent communication skills with the ability to educate patients effectively.
